The Division of Waste Management and Radiation Control has completed its review of the 3'd Quarter
201 5 Chloroform Monitoring Report.
The review was based on compliance with the following:
l. White Mesa Uranium Mill Ground Water Quality Assurance Plan (QAP). Revision 7.2,
dated June 6, 2012.2. Groundwater Discharge Permit No. UGW3700A4 Germi0. Permit version that was
signed on August 24,2012.3. White Mesa Uranium Mill Groundwater Corrective Action Plan (GCAP) Revision 0,
dated September 14, 2015.
Based on this review, the Report is in compliance with the Permit, QAP, and the GCAP. This letter
constitutes a closeout of the 3'o Quarter 2015 Chloroform Monitoring Report. If you have any questions,
