The Division of Waste Management and Radiation Control has completed its review of the 2014 Annual
Seep and Springs Sampling Report. The review was based on the following:
l. White Mesa Uranium Mill Ground Water Quality Assurance Plan (QAP), Revision 7.2, dated June 6,
2012.
2. Groundwater Discharge Permit No. UGW370004 ("Permit"), signed on August 24,2012.
3. Sampling Plan for Seeps and Springs In the Vicinity of the White Mesa Uranium Mill, Revision 0
(Plan).
The Division has determined that the Report was in compliance with the Permit, QAP and Plan. This letter
constitutes a closeout of the 2014 Annual Seep and Springs Sampling Report.
