Dear Ms. Weinel:
On June 10, 2015 the Utah Division of Radiation Control c·DRC') received by e-mail from EFR
updated weekly and monthly inspections forms ("forms" ) that added the new chloroform
monitoring well (TW4-37) for the chloroform and nitrate pumping systems at the White Mesa
Uranium Mill. Based on the DRC review these forms are approved.
