Dear Ms. Weinel:
The Utah Division of Radiation Control ("DRC") has completed the review of the 2nd, 3rd and
4th Quarter 2014 Chloroform Monitoring Reports for the Chloroform Contamination
Investigation, Energy Fuels Resources (USA) Inc. ("EFR") White Mesa Uranium Mill, near
Blanding, Utah.
The review was based on compliance with the Ground Water Quality Discharge Permit No.
UGW370004 ("Permit") and the EFR Quality Assurance Plan ("QAP"). The DRC review found
that the contents of the Reports comply with the applicable Permit and QAP requirements.
This letter constitutes a closeout ofthe 2nd, 3rd and 4th Quarter 2014 Chloroform Monitoring
Reports for the Chloroform Contamination Investigation.
