May 7,2012 Corrective Action Plan ("CAP") for Nitrate, White Mesa Uranium Mill,
Dear Mr. Lundberg:
Pursuant to our teleconference on March 3, 2015, Energy Fuels Resources (USA) Inc. ("EFRI") is submitting
the As-Built drawings for the Ammonium Sulfate Cover specified in the Nitrate CAP, dated May 7, 2012. The
attached As-Built drawings in Attachment 1 show the work completed to date. EFRI submitted the design
drawings for the work completed to date on October 3, 2014. DRC approved the design on October 16, 2014
and EFRI submitted the installation notification on November 6, 2014.
As requested during the March 3, 2015 teleconference, EFRI is also submitting the design for the concrete cap
for the existing concrete pad directly under the ammonium sulfate tanks. This design was not included in the
previous submissions and is being submitted at this time for your review and approval. The design is included
in Attachment 2. Upon receipt of DRC approval, EFRI will complete the concrete capping activities within 60
days of approval. Any schedule changes will be approved by the Director. EFRI will provide DRC with at least
fourteen calendar days notice for the construction of the cap.
It is important to note that the Nitrate CAP stated that a curbed concrete pad would be installed in the
ammonium sulfate tank area. Due to the significant expansion of the concrete in the area, the curb is no longer
practical and would interrupt and interfere with the drainage system and as such the curb initially contemplated
has been removed.
