Dear Mr. Lundberg:
Consistent with the Stipulated Consent Order Docket No. UGW12-04, Part E.l, Energy Fuels
Resources (USA) Inc. ("EFRI") is providing the Division of Radiation Control ("DRC") with at least
fourteen days notice for the construction of the Ammonium Sulfate Cover specified in the Nitrate
CAP, dated May 7, 2012. EFRI plans to initiate site assessment activities on Monday, November 17,
2014 with actual site preparation and construction activities commencing on November 20, 2014.
