The Utah Division of Radiation Control ("DRC") has completed the review of the November 18, 2013 White
Mesa Uranium Mill 3rd Quarter 2013 Nitrate Monitoring Report ("3rd Quarter 2013"), February 21, 2013
White Mesa Uranium Mill 4th Quarter 2013 Nitrate Monitoring Report ("4th Quarter 2013") and the May 19
2014, White Mesa Uranium Mill 1st Quarter 2014 Nitrate Monitoring Report ("1st Quarter 2014") for the
Nitrate Contamination Investigation at the EFR White Mesa Uranium Mill.
The review was based on compliance with the: 1) Energy Fuels Resources (USA) Inc., June 7, 2012, White
Mesa Uranium Mill, Ground Water Monitoring Quality Assurance Plan, Revision 7.2 ("QAP") and, 2) May 7,
2012, Corrective Action Plan for Nitrate White Mesa Uranium Mill near Blanding, Utah ("CAP").
The DRC review found that the contents ofthe 3rd Quarter 2013, 4th Quarter 2013, and 1st Quarter 2014
reports comply with the applicable QAP and CAP requirements. This letter constitutes a closeout of these
reports for the Nitrate Contamination Investigation.
