Methylene chlonde, Chlonde and Nitrate plus Nitnte as Nitrogen Use of analytical methods is consistent with the requirements of the QAP requirements QAQC Holding Times The method holding bmes for each parameter for samples collected at each monitor well were received and analyzed within the required holding times in the QAP However, one of the three tnp blank samples exceeded holding times for the VOC analysis EFR addressed this issue with problem with the following corrective action "Identification and Defimtion of the Problem The Trip Blank sample collected on 9/10/2012 was analyzed by the laboratory outside of the required holding time In a review of the data, EFR determined the Trip Blank was received by the laboratory on 9/14/2012 and not analyzed until 9/26/2012, which resulted in the data being outside the required holding time Determination of a Corrective Action to Eliminate the Problem The Trip Blank sample, which was analyzed outside of required holding time, was a result of a laboratory error The samples were received with more than sufficient time to analyze all samples within the required holding time EFR has determined this issue is due to laboratory error and not due to any actions or inactions by EFR As discussed above, EFR has initiated contract negotiations with an alternate Utah certified analytical laboratory and expects to make the change for the 1st quarter 2012 sampling event Assisnins and Accepting Responsibility for Implementins the Corrective Action Trip Blank Holding Times will be the responsibility of the QA manager to review data from the alternate Utah certified laboratory to assure completion of analysis within the method specified holding times Implementing the Corrective Action and Evaluatin2 Effectiveness EFR has contacted the current laboratory to make them aware of the issues In addition, EFR has initiated the process to find and change analytical laboratories The change to a new analytical laboratory is expected to occur pnor to the 1st quarter 2013 sampling event Once a new laboratory is identified, the sample receipt data will be assessed as samples are received at the new laboratory to determine if any further action is necessary Verifvins That the Corrective Action Has Eliminated the Problem Verification that samples are analyzed within the required holding times, will occur during the assessment of data collected from the first quarter 2013 sampling event " Page 3 DRC accepts EFR corrective action samples for exceeding holding times and no further action is required at this bme Receipt Temperature All samples were received withm the required,QAP temperature limit of 6° C Detection Limits All analytes were measured and reported to the required reporting limits, except 22 sets (21 samples and 2 duplicates) of sample results had the reporting limit raised for at least one analyte due to matnx interference and/or sample dilution In all cases the reported value for the analyte was higher than the increased detection limit Sample Duplicates Two duplicate samples were collected Well MW-32 and its duplicate sample TW4-65 and TW4-25 and Its duplicate sample TW4-70 All analytical results for both the sample/duplicate pairs were within the 20% acceptance limits as required in the QAP (see Table 2 below) Groundwater Head Monitoring Depth to groundwater was measured m the following wells and/or piezometers, as required in Part IE 3 of the Permit • The quarterly groundwater compliance monitonng wells • Existing monitonng well MW-4 and all of the temporary chloroform investigation wells • Piezometers P-1, P-2, P-3, P-4 and P-5 •MW-20andMW-22 • Nitrate monitonng wells • The DR piezometers which were installed dunng the Southwest Hydrologic Investigation In addition, weekly and monthly depth to groundwater measurements were taken in MW-4, MW-26, TW4- 4,TW4-19,andTW4-20 Mass Removed Chloroform removal was first estimated from the first quarter 2007 Since that esbmation the mass removed by each pumping well for each quarter has been calculated Esbmated mass removed in 3rd quarter 2012 is 6 6 lbs (see Table 3 below) Reporting The Report included the followmg information as required m the QAP a) Introduction b) Sampling and Monitonng Plan -Descnption of monitor wells -Descnption of sampling methodology, equipment and decontammabon procedures -Identify all quality assurance samples, e g tnp blanks, equipment blanks, duplicate samples c) Data Interpretabon Page 5 Monitor Wells with Chloroform Concentrations of <70ug/L There are nine monitor wells that have chloroform concentrabons of <70pg/L Monitor well TW4-5, TW4- 6, TW4-8, TW4-9, TW4-16, TW4-24, TW4-26, and TW4-27 have decreasing trends Monitor well TW4- 18 appears to have a slightly increasing trend with chloroform concentrations rangmg between 29 and 38 pg/L for the last 10 monitonng events (see Table 1 below) Pumping Wells Chloroform Concentrations All four pumping wells exceed chloroform concentrations of 70pg/L Pumping wells MW-4, TW4-19, and TW4-20 show decreasing trends and pumping well MW-26 show an increasing trend (see Table 1 below) Monitor Wells Below the Detection Limit The following monitor wells are below laboratory detection limit for chloroform (10 pg/L) MW-32, TW4-3, TW4-12, TW4-13, TW4-14, TW4-23, and TW4-25 (see Table 1 below) Nitrate. Nitrate + Nitnte ("nitrate") Concentrations Monitor wells TW4-20, TW4-21, TW4-22, TW4-24, and TW4-25 all exceed the nitrate GWQS of 10 mg/1 All of these nitrate exceedances for these wells appear to be a result a of nitrate contaminant plume up- gradient of the chloroform contaminant plume This nitrate contaminant plume is imder investigation as required by the January 2009 Stipulated Consent Agreement, UDEQ Docket No UGW-09-03 Dunng the review of this Report it was foimd that nitrate has exceeded its GWQS (10 mg/L) in well TW4- 12 in the 3"* quarter 2012 monitonng event (13 mg/1) and well TW4-27 exceeded nitrate GWQS m the last three consecubve quarterly monitoring events (1st quarter 2012 = 24 mg/1, 2nd quarter 2012=41 mg/1, 3rd quarter 2012= 37mg/l) These nitrate exceedances are not mentioned in the Report however, were noted in secbon 4 2 3 of EFR 3rd quarter 2012 nitrate monitonng report with the following statement Section 4 2 3 "Note that samples from chloroform monitoring well TW4-27 and TW 4-12 have measured nitrate concentrations that are higher than 10 mg/L However, nitrate concentrations in these wells are clearly separated from the nitrate/chloride plume at the Mill site by many wells that are below 10 mg/L or are nondetect for nitrate, including but not limited to, TW4-3, TW4-9, TW4-10, TW4-23, MW-32 and TW4-16 Thus, nitrate in TW4-27 and TW4-12 appear to be isolated from the plume at the Mill site, in the same way that the relatively small nitrate plumes at TWN-9 and TWN-17 are isolated from the plume at the Mill site " DRC staff agrees with EFR statement above with the exception that a further mvestigabon is necessary to determine the source and extent of the nitrate contaminabon at TW4-12 and TW4-27 Therefore, EFR must be required to submit a contammabon invesbgatibn plan to invesbgate nitrate exceeding the GWQS for at and m the vicinity of wells TW4-12 and TW4-27 Page 4 -Interpretation of groundwater levels, gradients, and flow direcbons Interpretabons will include a discussion on 1) A current site groundwater contour map, 2) hydrographs to show groundwater elevation in each monitor well over time, 3) depth to groundwater measured and groundwater elevation firom each monitor well summanzed in a data table, that includes histonc groundwater level data for each well, and 4) an evaluation of the effecbveness of hydraulic capture of all contaminants of concern -Interpretation of all analytical results for each well, including a discussion on 1) a current chloroform isoconcentration map with one of the isoconcentration lines showing the 70 pg/L boundary, 2) graphs showmg chloroform concentration trends in each well through time and, 3) analybcal results for each well summanzed in a data table, that mcludes histonc analytical results for each well -Calculate chloroform mass removed by pumping wells Calculations would mclude 1) total histonc chloroform mass removed, 2) total histonc chloroform mass removed for each pumping well, 3) total chloroform mass removed for the quarter and, 4) total chloroform mass removed fi-om each pumping well for the quarter ' d) Conclusions and Recommendations e) Electromc copy of all laboratory results for Chloroform Investigation monitonng conducted dunng the quarter f) Copies of DUSA field records, laboratory reports and chain of custody forms Conclusions on Compliance with the Permit and QAP The Report appeared to be m compliance with the Permit and QAP with the exception of a tnp blank exceeding holding bmes for VOC analysis However, as stated above, DRC accepts EFR correcbve action for samples exceeding holding times and no further acbon is required at this bme Evaluation of the Chloroform Contaminate Plume and Its Associated Contaminants Bounding Chloroform Plume It appears that the chloroform contaminate plume and associated contaminants, carbon tetrachlonde, chloromethane, and methylene chlonde is bound in all direcbons by wells that have a chloroform concentrabon of <70 [ig/L (GWQS = 70 pg/L) Therefore, no addibonal wells, at this time, need to be installed downgradient, across, or upgradient of the chloroform contaminate plume to demonstrate the plume is bound withm a 70 pg/L isoconcentrabon line Captunng Chloroform Plume In addition, it appears that the pumping wells (MW-4, MW-26, TW4-4, TW4-19, and TW4-20), at this time, are captunng groundwater with chloroform concentrabons exceeding 70 pg/L Monitor Wells Exceeding Chloroform Concentrabons of 70ug/L There are seven monitor wells exceeding chloroform concentrations of 70pg/L Monitor well TW4-1, TW4-2, TW4-7, TW4-10, TW4-11, and TW4-22 have decreasing trends Monitor well TW4-21 has an increasing trend (see Table 1 below) Page 6 References Denison Mines (USA) Corp, June
