Dear Ms Tischler.
On September 20 and 21, 2012, DRC staff conducted a field inspection of the physical facilities
involved in tailings and waste disposal at the White Mesa Mill The inspection focused on the
physical health and performance of the facilities with respect to the Discharge Minimization
Technology Plan and the approved implementation of best available technology at the site The
following findings were noted
1 The facilities appeared to be operating within expected parameters Of particular note, the
Cell 1 and Roberts Pond liner repairs appear complete Grooming of the protective soil
cover over the repaired Cell 1 liner had not been completed at the time of the inspection
2 Minor nil erosion has emerged on the extenor of the dikes on the south of Cells 4A and
43 This should be conected before the nils enlarge and endanger the stability ofthe
dikes See the attached Photo 1
3 The movement monitors along the south side of Cell 4A are in the center of the road atop
the dike Grooming activities endanger the monitonng points DRC recommends
relocating those monitonng points to the outer shoulder of the dike, as was done for Cell
4B, to prevent disturbing the monitors See the attached Photo 2

4 Two leaking flange joints were identified on a solution retum line The leaking joints were
located over Tailings Cell 2 Repair crews repaired the leaks and moved affected soil to a
disposal site in Cell 3 See the attached Photo 3
