The review was based on compliance with the Ground Water Quality Discharge Permit Number UGW370004
(hereafter "Permif') and the DUSA Quality Assurance Plan (hereafter "QAP") The review found contents of the
Report to comply with the applicable Perrmt and QAP ^
In Section 1 0 (Intioduction) ofthe Report, DUSA refers to the Notice of Violation and Groundwater Corrective
Action Order (hereafter "NOV") Docket Number as "UGQ-20-01" This Docket Number is mcorrect Accordmg to
the letter dated August 23, 1999 that the Department of Envuronmental Quality Division of Radiation Conti-ol issued
to DUSA, the conect NOV Docket Number is "UGW-20-01" (DRC 1999) Please conect NOV Docket Number for
