On September 1 2011 the Utah Division of Radiation Control (DRC) received the DMT Performance
Standards Momtonng Report and Cell 4A and Cell 4B Performance Standards Monitoring Report (Report
Dated August 31 2011) for the DUSA White Mesa Mill near Blanding Utah The DRC reviewed the
Report and issued a letter of Findings Confirmatory Action and Notice of Enforcement Discretion on
October 27 2011 which documented DUSA s commitment to provide several items of data in a revised
Report document DUSA provided the requested revised Report on November 14 2011
The revised report contained additional monitonng data for the Cell 2 Slimes Drain pool surface
elevations for all tailings cells and Roberts Pond tailings beach area and elevation survey data and Cells 1
and 3 leak detection system monitoring results These data support claims made in the onginal draft of the
Report that DUSA had complied with requirements of the DMT and BAT plans in force at the time
Based upon this finding the DRC considers the review of the 2"^* Quarter Report closed
