>rd Review of the 3 Quarter 2011 Chloroform Monitonng Report for the Chloroform
Contamination Investigation Denison Mines (USA) Corp (hereafter DUSA )
White Mesa Uranium Mill near Blanding Utah
The Division of Radiation Control (hereafter DRC ) has reviewed 3'^^ Quarter 2011 Chloroform
Monitonng Report (hereafter Report ) for the Chloroform Contamination Investigation For the
review of these reports the following regulatory enforcement documents were used
• DUSA was issued Ground Water Quality Discharge Permit No UGW370004 (hereafter
Penrnt ) on March 8 2005 last revised on January 20 2010
• The DUSA Quality Assurance Plan (hereafter QAP ) March 22 2010 (Revision 6 0)
Review
The 3rd Report appeared to be in compliance with the Permit and QAP with the exception of the
not meeting the reporting requirements found m section 6(c)(3^'^ bullet) of Appendix A of the
QAP This section of the QAP requires
Calculate chloroform mass removal by pumping wells Calculations would include 1) total
historic chloroform mass removed 2) total historic chloroform mass removed for each pumping
well 3) total chloroform mass removed for the quarter and 4) total chloroform mass removed
from each pumpmg well for the quarter
DUSA reported incorrect total mass removal for the quarter for pumping wells IVTW 24 TW4 4
TW4 19 and TW4 20 and the total mass removal for the quarter Using volumes (gallons)
pumped and chloroform concentrations reported in the Report DRC calculated 8 1 lbs of
chloroform removed for the quarter (see Table la below) and DUSA reported (calculated) 13 2

lbs removed It appears that DUSA rmstakenly used the 2"^* quarter 2011 chloroform
concentration for MW 24 TW4 4 TW4 19 and TW4 20 to calculate the chloroform mass
removal for the 3'^'' quarter 2011 (see Table lb below)

DRC staff recommends that no notice of violation be issued but instead issue DUSA a notice of
enforcement discretion informing them of the error and requesting that they make the necessary
corrections to accurately report chloroform mass removal in future chloroform monitonng reports
