Box 144810 Salt Lake City, UT 84114-4820 %S ^..^ ..^o vwSA) Corp. 1050 ffth ^reet, Suite 950 Denver, CO 80265 USA Tel : 303 628-7798 Fax:303 389-4125 www.denlsonmines.com Dear Mr. Lundberg: Re: State of Utah Ground Water Discharge Permit ("the Permit") No. UGW370004 White Mesa Uranium Mill - Installation Report Pursuant to Part I.F.6 of the Permit This letter transmits the As-Built Report for Denison Mines (USA) Corp's ("Denison's") perched groundwater monitoring well TW4-27, as required by the May 26, 2011 Division of Radiation Control ("DRC") Request for Additional Information ("RFI"), and as delineated in the Final Denison Work Plan and Schedule to Drill and install Well TW4-27, submitted to DRC on October 3, 2011. The May 26. 2011 RFi (received June 2, 2011) required the following: Submit for tlie Executive Secretary approval a plar) of action and worl< schedule to complete the installation of one or more wells in locations hydraulically down gradient of wells TW4-4 and TW4-6 to demonstrate the plume is bound and to assure that the plume is hydraulically being captured in this vicinity. In conjunction with the existing wells, these new well(s) must show on a map: The 70 ug/I chloroform isoconcentration line remains hydraulically up gradient to the new well(s) and, • Groundwater contour lines must show the new well(s) are hydraulically down gradient to TW4-4 andTW4-6. Denison submitted the required information On June 17, 2011. Further comments and negotiations were completed and the FinaiPian and Schedule were submitted on October 3, 2011. TW4-27 was installed on October 18, 2011. Installation History and Conformance with GWDP Requirements Denison did provide 15-day written notification to DRC in advance of installation of TW4-27. Per the requirements of the Plan and Schedule, monitoring well TW4-27 was installed hydraulically downgradient of wells TW4-4 and TW4-6 to ensure the chloroform plume is bounded. Based on the estimated Letter to Rusty Lundberg TW4-27 Novemebr 28, 2011 Page 2 perched water elevation at TW4-27, TW4-27 is located downgradient ofrW4-6 and TW4-26, and TW4-14 is located downgradient of TW4-4 and TW4-6. TW4-14 and TW4-27 are therefore properly located to bound the chloroform plume to the east of TW4-4, TW4-6, and TW4-26. The enclosed As-Built Report in Attachment 1 includes the items required for As-Built Reports in the Permit Part I.F.6, and is being submitted for TW4-27. Aquifer hydraulic testing was completed for TW4-27 November 8, 2011. TW4-27 is underdevelopment by surging and bailing. Development is slow due to low permeability conditions. Development by overpunnping will be completed afterthe surging and bailing has been completed. Piease contact the undersigned if you have any questions or require any further information.
