SUBJECT: Denison Mines (USA) Corp. ("DUSA") Chloroform Contamination Investigation,
Installation of .Monitor well TW4-26 Closeout Letter
Dear Mr. Frydenlund:
The Division of Radiation Control ("DRC") has reviewed the DUSA July 26, 2010 As Built Report for
chloroform Monitor Well TW4-26 and the DUSA September 20, 2010 Hydraulic Testing ofTW4-4, rW4-6,
and TW4-26 White Mesa Uranium Mill July 2010. Based on our review of both of these reports it appears
that DUSA has met the requirements outlined in DUSA February 18, 2010 Plan of Action and Work
Schedule for monitor well TW4-26. This letter constitutes a closeout of the installation of monitor well
TW4-26.
