Date: 12/16/2010 9:10 AM Subject: FW: Requirement for Cell 4B: GWDP Part I.H.11 Dave, The cushion geotextile flap (sewn seam) can be placed on either side of the header pipe as it is a feature not related to the performance of the filter geotextile other than to seal the edge. The cushion geotextile and overlying woven geotextile currently conform to the intent of the design drawings and the additionai requirements outlined in the permit. The sand bags will be rearranged to provide continuous coverage (end to end, not side to side) along both the woven geotextile flaps. We will send photos of the completed sand bag rearrangement v^rhen complete. Please let us know if you have questions or need additional Information. As a courtesy, we wish to notify you that we have discovered the subject requirements in the Ground Water Discharge Permit (GWDP), which affect the Ceil 4B project. Part I.H.11 of the GWDP reads as follows: "11. Corrections to Engineering Dravinng Sheet 6 of 8 - prior to construction of Cell 4B, the Permittee shall revise Cross-section B-7 presented in Engineering Drawing Sheet 6 of 8 and any related technical specifications, to reflect the following changes: (a) Both cushion geotextile material flaps must be extended at least 1-foot laterally beyond the sewn seam at the right (north) side of toe of the drainage aggregate windrow, and (b) Sandbags placed on both the right (north) and left (south) side of the drainage aggregate windrow, must be placed and abutted continuously along the length of the vflndrow." We are not aware if the above changes are included on the current as-built drawings ih the CQA Report. Also, it appears item 11 .b. above has not been implemented in the field: Please assure the following; 1. If not already included, please submit the above drawing corrections as addenda In the Cell 4B Construction Quality Assurance Report. 2. Implement the requirements of item 11 .b in the work, and provide written documentation, with dated photographs, certified by a Utah-licensed Professional Engineer, that demonstrates completion of this requirement, and include it as part of the Ci3A report. The subject items, and others, are required to be completed for DUSA to receive DRC authorization to operate Cell 4B. This email has been provided as a early notice regarding the subject, and because we believe DUSA is desirous to rapidly complete all requirements needed to begin operation of Cell 4B.
