Chloroform Concentrations Exceeding the Groundwater Quality Standard in Monitor Well TW4-6. Submittal of a Plan of Acfion and Work Schedule. White Mesa Uranium Mill, - Conditional Approval Letter. Dear Mr. Frydenlund: The Division of Radiation Control (hereafter "DRC") has reviewed the February 18, 2010 Plan of Acfion and Work Schedule (hereafter "Plan") that was submitted by Dennison Mines (USA) Corp. (hereafter "DUSA") to have hydraulic containment of the chloroform concentrations at 7^4-6. The submitted Plan is in response to a February II, 2010 conference call with DRC and DUSA that discuss DUSA's December 31, 2009 proposed plan and acfion and schedule. The conclusions reached in this conference call were confirmed in an email dated February 11, 2010 from DRC to DUSA. Based on our review the Plan is condifionally approved with the following modificafions: 1. Modify Item 3 of the Plan to read (added wording is underlined deleted wording is strikeout): Denison will install new chloroform monitoring well TW4-26 at the location indicated on Figure I attached to Hydro Geo Chem's February 16, 2010 letter report. This well and all other new monitorins wellfs) will he pennanent and constructed in accordance with UAC R 317-6-63(l)(6)and shall comply with Part I.F.6 ofthe DUSA Ground Water Discharge Permii No. UGW370004. Modify Hem 4 ofthe Plan to read (added wording is underiined deleted wording is strikeout): Denison will aquifer le.st TW4-4, T\V4-6, and Ty\'4-26, on or before August 1, 2010, lo a) Calculate penneabilities ond hydraulic conductivities: b) Calculate local groundwater velocities; and c) Detennine an endent pumping system at each pumping well location. Such aquifer testing will be described in a report that will be submitted lo the Executive Secretary on or before Sepiember 30, 2010. Modify Item 6 to read: Afier submittal ofthe 4*'^ quarter, 2010 chloroform monitoring report, if the Execufive Secretary determines for any reason that pumping TW4-4 is not effecfively reducing the chloroform concentration in TW4-6 DUSA will begin pumping TW4-6 wilh in 30 days of written notice from the Execuiive Secretary. 4. Remove actions in Item 7 as a result of the wording changes in Item 6. 5. Remove aclions in Item 9 as a result of the wording changes in Item 6. Tn our conference call on April 7, 2010 it was agreed that within 30 days ofthe receipt ofthis letter DUSA would provide, in electronic PDF format, both a final copy of the Plan and a copy of the Plan in redline strikeout format showing the above modifications requested.
