2011 Wastewater Monitoring Event for the Denison Mines (USA) Corp (hereafter
DUSA ) White Mesa Uranium Mill near Blanding Utah
On August 24 2011 the Division of Radiation Control (hereafter DRC ) staff Dean Henderson
observed DUSA staff perform the 2011 wastewater monitonng event and collected a split sample
from the wastewater m Tailing Cell 3 DUSA staff performing the sampling was Ryan Palmer
and Tanner Holliday
After observing the DUSA staff perform the wastewater sampling it appears they followed the
currently approved Tailings and Slimes Drain Sampling Plan revision 0 dated November 20
2008
Tailing Cells 1 3 4A and 4B LDS pumps were all able to be manually tumed on to demonstrate
that the pumps were operational However the indicator light on the control panel (see Figure 3)
could not be manually tumed on Mr Palmer said that this light does work however he just did
not know how to manually tum it on and will leam how I told him to figure out and when DRC
staff comes to the White Mesa Mill for the tailings groundwater sampling event in October 2011
DUSA staff could then demonstrate that the light works for each LDS system This request by
DRC staff was follow up by email (see Attachment 1)
Also each LDS was checked for water The LDS with transducers (tailing cells 1 3 4A and 4B)
showed a water level of 0 mches on the digital water level monitors on the control panels In
Tailings Cell 2 LDS measurement of water on the dip stick was about 1 inch Using the dip stick
It also felt like there was some debris at the bottom It is recommended that this LDS also be
video logged to see what debns may exist at the bottom that needs removal Should debns need
to be removed the water at that time could also be removed perhaps by suction
It was also observed that tailings cells 1 and 3 LDS have been designed for video log access (see
Figures 1 3 and 4)

I was at the White Mesa Mill on August 24, 2011 to observe the wastewater sampling event I also inspected
the tailing cells LDS I had Ryan manually tum on the pumps for each LDS (except for tailing cell 2) I also
asked him to manually turn on the red indicator light for each LDS but he at that time did not know how to
turn them on I told him to let me know when he figures out how to turn this light on and when the DRC staff
IS down again for the tailings wells monitormg event m October 2011 he could demonstrate that the lights
works
