Dear Ms. Weinel:
The Division of Waste Management and Radiation Control has completed its review of Energy Fuels
Resources’ (EFR) updated weekly and monthly inspection forms to add two new chloroform wells
(TW4-40 and TW4-41) for the chloroform and nitrate pumping systems (forms).

The proposed forms were included in the April 9, 2018 EFR letter, “Transmittal of Energy Fuels Resources (USA) Inc. (“EFRI”) Updated Weekly and Monthly Inspection Forms for the Chloroform and
Nitrate Pumping Systems at the White Mesa Uranium Mill.”
