Ground Water Quality Discharge Permit Number UGW370004 Dear Ms. Weinel:

The Division of Waste Management and Radiation Control has completed its review of the November
17, 2016, 3rd Quarter 2016 Chloroform Monitoring Report and the February 22, 2017, 4th Quarter 2016 Chloroform Monitoring Report (Reports). The review was based on compliance with the following:

1. White Mesa Uranium Mill Ground Water Quality Assurance Plan (QAP). Revision 7.2, dated June
6, 2012.
2. Groundwater Discharge Permit No. UGW370004 (Permit), version signed on August 24, 2012. 3. White Mesa Uranium Mill Groundwater Corrective Action Plan (GCAP) Revision 0,
dated September 14, 2015.

Based on the review, the Reports are in compliance with the Permit, QAP and GCAP. This letter
constitutes a closeout of the Reports.
