Corrective Action Plan for Nitrate White Mesa Uranium Mill - May 7, 2012
Closeout Letter Dear Ms. Weinel:

The Division of Waste Management and Radiation Control has completed its review of the May 17,
2016, White Mesa Uranium Mill 1st Quarter, 2016 Nitrate Monitoring Report and the August 16, 2016, White Mesa Uranium Mill 2nd Quarter, 2016 Nitrate Monitoring Report for the Nitrate Contamination
Investigation.

The purpose of the review was to determine compliance with:
1. The Corrective Action Plan (CAP) for Nitrate, White Mesa Uranium Mill, May 7, 2012,
approved December 12, 2012.
2. The EFR Quality Assurance Plan (QAP), June 7, 2012, Revision. 7.2.

Based on its review, the Division concludes that both Reports are in compliance with the CAP and QAP. This letter constitutes a closeout of the 1st Quarter, 2016 Nitrate Monitoring Report and the 2nd Quarter,
2016 Nitrate Monitoring Report.
