Closeout Letter Dear Ms. Weinel:

The Division of Waste Management and Radiation Control has completed its review of the
May 19, 2016 1st Quarter 2016 Chloroform Monitoring Report and the August 18, 2016, 2nd Quarter 2016 Chloroform Monitoring (Reports).

The purpose of the review was to determine compliance with the:

1. White Mesa Uranium Mill Ground Water Quality Assurance Plan (QAP), Revision 7.2, dated June 6, 2012.
2. Groundwater Discharge Permit No. UGW370004 (Permit), signed on August 24, 2012.
3. White Mesa Uranium Mill Groundwater Corrective Action Plan (GCAP) Revision 0, dated
September 14, 2015.
The Division has determined that the Reports are in compliance with the Permit, QAP and GCAP. This letter constitutes a closeout of the May 19, 2016, 1st Quarter 2016 Chloroform Monitoring Report and
the August 18, 2016 2nd Quarter 2016 Chloroform Monitoring Report.
