Observations from the inspection were discussed with Dave Turk and Ronnie Nieves at the closeout meeting. The inspection was an examination of the activities conducted in your facility as they relate to compliance with the Utah Radiation Control Rules and the conditions of the Mills RML. The inspection consisted of an examination of representative records, interviews of personnel, and observations by the inspector. During the closeout meeting the DRC inspector made reconunendations regarding items for improvement. These recommendations can be found at the end of the inspection report under conclusions and recommendations. Enclosed is the inspection report regarding this inspection for your review. Inspection Summary The inspection consisted of reviewing the implementation and documentation of the revised (Rev. 9/28/09) Respiratory Protection Program as it pertains to the requirements found in the UAC R313-15-703 and the NRC Regulatory Guide 8.15 as referenced in 2007 RML renewal. The inspection included a review of documentation, an inspection of respiratory protection equipment and facilities, employee interviews, a RSO interview, and a mill tour. The following discussion provides more detail of the specific items. During the inspection it was observed that the Mill was using an older version of the Respiratory Protection Program and the inspection was adjusted accordingly. «-.*T..^ 1 of Page 4 C:\DOCLlME-lUhullqui\LOCALS-rTaTip\XPgrpwise\lnspcction Report RADMOD-RPP-02 12-2-09 RJ.doc Item 1, Documentation: The inspector reviewed documents that were associated with the Respiratory Protection Program. The inspector was able to confirm that all procedures required by UAC R313-15-703 and NRC Regulatory Guide 8.15 were available at the Mill. The inspector was^ able to verify that training, medical and fit-test records for each respirator user were being maintained. The inspector verified training records for radiation, safety and environmental technicians regarding the Respiratory Protection Program. In addition, the inspector reviewed respirator survey sheets and was able to confirm that they were filled out with all relative information required and appropriate survey units were being used. Also the inspector reviewed the Respirator Issuance logs and found them complete and legible. The inspector asked if a copy of the Respiratory Protection Program was kept in the respirator room and the answer was no. The inspector asked if there was a list of all qualified respirator users with the size of respirator that they are fit tested for was kept in the respirator room and the answer was no. Deficiencies: RML UTl 900479 License Condition 9.6 states "An up-to-date copy of each written procedure shall be kept in the mill area to which it applies." An up-to-date copy of the Respiratory Protection Program is not kept in the respirator room. NRC Regulatory Guide 8.15 Section 4.5 Control and Issuance states "Persotmel who issue respirators must ensure that each person issued a respirator has been medically screened, trained, and (for face-sealing devices) fit-tested within the period prescribed. Persons may only be issued face-sealing respirators for which they have been fit- tested (i.e., same make, model, style, and size)." An up-to-date list of all qualified respirator users at the Mill with fu-test information is not located in the respirator room. Item 2. Respiratory Protection Program Equipment and Facilities: The inspector inspected the Mills respirator room. The Mill uses MSA brand full face "Advantage" air purified respirators with "GME-PI OO" combination cartridges which are NIOSH certified. The inspector asked the Radiation Coordinator to demonstrate how respirators are decontaminated and surveyed. Dirty respirators are inspected for damage by radiation technicians, decontaminated using dishwashers with dishwasher detergent but not taken apart, dried in a respirator drier, cleaned with Fantastic spray cleaner and a radiological survey is performed with disc smears. The inspector asked if the radiation technicians used disposable gloves when working in the respirator room, the Mills radiation coordinator answered yes but the gloves are not kept in the respirator room. The inspector inspected ten decontaminated respirators for cleanliness and damage. The inspector reviewed the respirator manufacturer's guidance for decontamination of respirators. The inspector observed the respirator storage cabinet in the respirator room was kept locked and all decontaminated respirators and parts were kept inside the cabinet in sealed plastic bags. The Mill performs two different types of fit-tests on respirator users. The first is a qualitative "smoke" test which is performed by radiation technicians each time a respirator is issued. The second is a quantitative "machine" fit test which is performed on all respirator users once a year to determine the size of respirator each user needs. The inspector observed a "smoke" fit test and participated in a mock *'machine" fit test. The inspector also observed a Mill employee properly perform a positive/negative seal check on the respirator that was assigned to him that day. Deficiencies; When the Inspector inspected the ten respirators it was observed that two respirators were badly scratched with cracked face shields where the air purifying cartridges are attached. Deficiencies: None Closeout Meeting Denison Mines: Dave Turk (RSO) Ronnie Nieves (Radiation Coordinator) Findings None Utah DRC: Ryan Johnson (Inspector) John Hultquist (Supervisor/Mspector) Conclusions and Recommendations During the close out meeting the DRC inspector made the following recommendations regarding items for improvement: 1. Additional training for Radiation, Safety and Environmental Technicians on the new version of the Respiratory Protection Program. 2. Put a copy of the Respiratory Protection Program in the Respirator Room. 3. Put a list of qualified respirator users in Respirator Room with fit-test information, 4. Contact respirator manufacturer (MSA) and obtain written confirmation that using dishwashers and dishwasher detergent to clean respirators will not degrade or damage respirators or follow manufacturer's instructions. RSO committed at the closeout meeting the inspector's recommendations will be completed and the training will be completed by December 7, 2009. The inspector received documentation fi-om the RSO that training was completed on December 7, 2009. Recommendation for Next Inspection 1. Fit testing must be performed with the facepiece operating in the negative pressure mode. (4) The licensee or registrant .shall advise each respirator user that the user may leave the area at any time for relief from respirator use in the event of equipment malfunction, physical or psychological distress, procedural or communication failure, significant deterioration of operating conditions, or any other conditions that might require such relief. (5) The licensee or registrant shall also consider limitations appropriate to the type and mode of use. When selecting respiratory devices the licensee shall provide for vision correction, adequate communication, low temperature work environments, and the concurrent use of other safety or radiological protection equipment. The licensee or registrant shall use equipment in such a way as not to interfere with the proper operation of the rcspirau>r. (8) The licensee shall ensure that no objects, materials or substances, such as facial hair, or any conditions that interfere with the face and facepiece seal or valve function, and that are under the control of the respirator wearer, are present between the skin of the wearer's face and the sealing surface of a tight-fitting respirator facepiece. (9) In estimating the dose to individuals from intake of airborne radioactive materials, the concentration of radioactive material in the air that is inhaled when respirators are wom is initially assumed to be the ambient concentration in air without respiratory protection, divided by the assigned protection factor. If the dose is later found to be greater than the estimated dose, the corrected value must be used. If the dose is later found to be less than the estimated dose, the corrected value may be used.
