SITE NAME: Silver Pointe Voluntary Cleanup Program (VCP) site
SITE LOCATION: Leeds, Utah
DATE OF SITE VISIT: 13 FEB 2024
PURPOSE OF SITE VISIT: Division of Environmental Response and Remediation (DERR) oversight of
scheduled repairs and activities pursuant to the SEP 2023 annual inspection report.
FIELD OBSERVATIONS: The DERR met the Silver Reef Investment Holdings, Inc. (SRIH) Site Engineer
(Rich White) near the Large Headframe at the Silver Pointe site on 13 FEB at ~10:45 am. Contaminated
soil from the future building lot (15) north of the Headframe, was being loaded into a dump truck bound
for the Washington County Landfill.
The DERR and SRIH then proceeded to the PCA6 area of the Silver Pointe site, an area within lots 31 and
32. The area had additional clean soil added in areas of erosion, and 2 inches of gravel added to reduce
the potential for erosion. We then proceeded to the PCA8 area on the east end of lot 4, and at the
eastern property boundary. Both areas appeared to have the specified additional clean cover to replace
what was eroded and the additional 2 inches of gravel to inhibit future erosion.

As the work proceeded during the rest of the day, SRIH and the DERR surveyed with field instruments
the excavated area at lot 15, collected aliquots for the confirmation sample and DEQ split, and oversaw
the additional clean soil placed at PCA12, PCA5, and the mine adit opening on lot 2.

The DERR walked the area of additional soil and gravel at the entrance to the mine found on lot 2, and
the extent of the cover was consistent with the recommendations in the Annual Inspection Report. The
DERR left the site at ~6:20 pm.
