INTRODUCTION
The Former Hooper Country Store facility is located on the northwest corner of the intersection of 5500
South and 5900 West in Hooper, Utah. The facility is the location of the former Hooper Country Store
which included an underground storage tank (UST) system from approximately 1980 through 1998. The
facility had three USTs: one 4,000-gallon diesel UST, and two 6,000-gallon gasoline USTs. Petroleum
hydrocarbon impacts to soil, groundwater, and soil gas at the facility are believed to be the results of
multiple, relatively small releases such as overfills and fueling system leaks. The facility has been the
subject of several phases of subsurface investigation, and several corrective measures including:
excavation and off-site disposal of impacted soils, treatment with Oxygen Releasing Compound (ORC®),
and chemical oxidation using Fenton’s reagent. Residual petroleum hydrocarbon impacts to groundwater
persist at concentrations in excess of the Utah Initial Screening Levels (ISLs) in off-site monitoring well
MW-18. The facility is tracked by the Utah Department of Environmental Quality (UDEQ), Division of
Environmental Response and Remediation (DERR) using Facility ID 1200214, and Release Site EFAK.
On October 22, 2018, Wasatch Environmental, Inc., (Wasatch) personnel completed monitoring well
abandonment activities on four monitoring wells (MW-11, MW-14, MW-15, MW-17) located at the Former
Hooper Country Store facility. This activity was documented for the DERR in a report dated November
20, 2018.
Groundwater monitoring was completed at the facility on November 4, 2022, and the results were
reported to the DERR in a report dated February 16, 2023. The report also included a request for
regulatory closure of the release. The DERR granted regulatory closure of the release in a “No Further
Action” (NFA) letter dated October 4, 2023. This report documents the abandonment of the seven
monitoring wells remaining at the facility, following receipt of the NFA letter.
MONITORING WELL ABANDONMENT ACTIVITIES
Wasatch obtained the necessary access agreements with the various property owners where the
remaining monitoring wells were located, prior to accessing the properties for well abandonment.
Following monitoring well abandonment, paved surfaces were patched with concrete or asphalt to match
the existing surface, and grass-covered areas were filled with soil, gravel, and grass seed.
On October 18 and 19, 2023, Vince Stansfield and Rachel Kubina of Wasatch completed monitoring well
abandonment activities of the seven remaining monitoring wells (MW-18, MW-3R, MW-AR, MW-BR, MW-
CR, MW-DR, and MW-E) located at the Former Hooper Country Store facility. The former monitoring well

locations are presented on Figure 1. All monitoring wells were less than 30 feet deep in total depth;
therefore, a licensed well driller was not required to complete the well abandonment activities.
With the exception of monitoring well MW-18, the well vaults, circular vault lids, and associated concrete
were removed. The casings were then cut down to 12 to 14 inches below ground surface (bgs). The
monitoring wells were filled with medium bentonite chips and hydrated with 5 gallons of potable water to
seal the monitoring well. At the request of Hooper City, after the well was sealed with bentonite and
hydrated as described above, the well vault for MW-18 was left in place (with the lid removed) and filled
with concrete to match the existing surface. The remainder of the excavations for the surface
completions were filled with concrete or asphalt to match the existing surface, in paved areas, or
backfilled with topsoil and manually compacted, in unpaved areas.

CONCLUSIONS
The monitoring wells at the facility have been properly abandoned and the pavement and ground surface
have been restored.
