Dear Mr. Eaton:
The Department of Environmental Quality, Division of Environmental Response and
Remediation (DERR) requests that Terracon Consultants, Inc. (Terracon) complete the enclosed
Work Assignment Number GZQ-59-TER, under Contract #216383. The enclosed Work
Assignment identifies the work necessary to continue petroleum vapor monitoring at the release site
and the adjacent Monticello Inn property.
Enclosed is an original copy of the Work Assignment and tasks to be completed. Please sign
and return electronically to the DERR.

Work Assignment: Continue Vapor Monitoring with Reporting
Brent Redd Property, located at 148 East Center Street, Monticello, Utah
Facility Identification No. 5000003, Release Site GZQ
The Consultant, Terracon, is requested to complete the following Work Assignment.
I. WORK OBJECTIVE:
To continue petroleum vapor monitoring at the release site and the adjacent Monticello Inn
property, by performing three quarterly sampling events with reports of findings.
II. TASKS
The Division of Environmental Response and Remediation (DERR) has identified the
following specific tasks which follows Terracon 's work plan and price submitted on March
18, 2024:
A. Scoping Meeting
Cost: $397.50
B. Indoor Air Sampling Event 1
Cost: $3,548.50
C. Vapor Monitoring Point Sampling Event 1
Cost: $1,651.50
D. Report of Actions and Invoicing Event 1
Cost: $3,299.00
E. Indoor Air Sampling Event 2
Cost: $3,548.50
F. Vapor Monitoring Point Sampling Event 2
Cost: $1,651.50
G. Report of Actions and Invoicing Event 2
Cost: $3,299.00
H. Indoor Air Sampling Event 3
Cost: $3,548.50
I. Vapor Monitoring Point Sampling Event 3
Cost: $1,651.50
J. Report of Actions and Invoicing Event 3
Cost: $3,299.00
Total Cost: $25,894.50
III. PRICE AND PAYMENT SCHEDULE:
A. The expense to the DERR for completing this Work Assignment shall not exceed
$25,894.50 , including administrative costs , contract management , project
management, and other associated costs.
B. Payment requests shall be submitted when tasks are completed, as outlined in the
attached Deliverable and Payment Schedule .
IV. SCHEDULE FOR COMPLETION OF WORK AND REPORTING
REQUIREMENTS
A. The DERR has set a deadline of April 2024 , and an end date of January 2025 for the
above-referenced tasks. Terracon is required to provide a draft report of findings
upon completion of the work as outlined above.
B. Following any DERR comments to the draft report, Terracon will be required to
provide a final report within 4 days .
