Dear Mr. Tao:
Terracon Consultants, Inc. (Terracon) has prepared this letter to document the abandonment
of the groundwater monitoring wells associated with Release Site JFK at the former Freeway
Chevron site in Washington, Utah.
Terracon prepared Work Plan JFK-19 to outline the scope of work assumed to be required to
abandon the existing monitoring wells. Work Plan JFK-19 included abandonment of 6 on-site
monitoring wells and 5 off-site monitoring wells associated with Release Site JFK.
Terracon mobilized to the site on January 17, 2024. Upon review of the newly developed site,
it was determined that all on-site monitoring wells had been destroyed during redevelopment
and re-grading of the site for the installation of the new convenience store and fueling facility.
Additionally, only 4 of the 5 off-site wells were still located off-site as MW-9 was destroyed
during re-grading of the area. A total of 4 off-site monitoring wells remained in the area (MW-
10, MW-13, MW-15 and MW-16R) and were abandoned following the protocol outlined in the
Work Plan.
All groundwater monitoring wells that were associated with the release were either destroyed
or properly abandoned. A photographic log of the site conditions and off-site monitoring well
abandonment procedures is attached.
