The file for this release has been reviewed by the Division of Environmental Response and
Remediation (DERR) project manager who has recommended that no further corrective action be
taken at this time. This no further action applies only to the specific Release OKA Site. This
recommendation is based on the information contained in the file for this release. That information
was supplied by you, your consultant, or another representative.

The information submitted indicates that any detectable petroleum contamination from this
release is not a threat to human health or the environment according to Utah Admin. Code rule
R311-211. In the future, if other evidence indicates a spread of contamination from this release
which may cause a threat, further corrective action may be required.

SUBJECT: Transfer of all hard copy information to a closed PST case file
Pilot Thomas Logistics, #140, 380 South 200 East, Roosevelt, Utah

I have reviewed the file for the Pilot Thomas Logistics, #140 (PTL) petroleum release, including
information obtained by the Division of Environmental Response and Remediation (DERR). My
recommendation for no further action (NFA), at PTL OKA
in its case file. I recommend that no Leaking Petroleum Storage Tank (LPST) file be maintained other
than the UST database LUST module file, and that all hardcopy information be filed in a closed LPST
case file. Because a Reporting and Remediation compliance schedule was sent to the responsible
party, a NFA letter will be sent. This recommendation for closeout is based on the fact that a diesel
spill, attributed to a customer overfill event, was abated and cleaned up within twenty-four hours.

File information justifying no further action includes the following: On February 19, 2024, a diesel
spill was called into the DERR by Aric Morton, Broadbent & Associates, Inc. Reportedly, an overfill
from a customer at the dispensing island occurred, spilling an estimated 20-gallons of diesel fuel. The
spill flowed down sloped pavement and was retained in an on-site catch basin. On-site personnel called
an environmental response company, Lincoln Environmental (Lincoln), who responded that day.
Adsorptive material was placed over the impacted asphalt area, and adsorptive pads and booms were
utilized at the catch basin. On February 20 th, Lincoln collected the pads and booms placed over the
catch basin, swept up the adsorptive material over the spill area, and placed a fresh boom in the catch
basin to capture any remnant fuel. A single 55-gallon drum contained the waste which was properly
disposed at the Little Mountain Landfill in Tremonton, Utah. The waste manifest was assigned EPA ID
#: UTR000006783. The recommendation for no further action at PTL is appropriate, as the noted
diesel spill was cleaned up within twenty-four hours, and does not present a threat to human health or
the environment.
