Dear Mr. Nielson and Mr. Tao:
This work plan and budget amendment has been prepared for additional costs due to actual quantities
of materials and work incurred or completed during soil abatement activities performed in March 2024
during abatement and site restoration. The additional cost of $180,848.60 is based on the general
contractor’s unit rates approved in Work Plan OIV-01 and excludes costs which would have been
incurred for the replacement of the UST system. Additional line items are included which are based on
actual billed amounts from vendor’s invoices. Two worksheets are attached which were used to
calculate quantities and rates applied as well as backup weight sheets.

The costs included in this work plan amendment were necessary for abatement activities and site
restoration. Release site OIV is Petroleum Storage Tank (PST) Fund eligible. Future PST Fund
payment requests will reference the Work Plan OIV-02 and Amendments No 1 and No 2.
A site map, attached, displays the excavation and resurfaced areas. The map also shows the areas
which were subtracted from the total resurfaced area for the purposes of Amendment No 2. The
attached work sheets include a budget spreadsheet and a general contractor’s cost sheet. The ladder
includes calculations for resurfacing areas eligible for PST Fund coverage and separates asphalt
replacement costs from concrete replacement costs. This was necessary since the entire resurfaced
area was done in concrete due to the lack of asphalt plants in the vicinity.
Based on the above adjustments the requested amount for Amendment No 2 is $180,848.60. The
signed work plan approval and agreement is also attached.
