Center in Logan, Utah - Facility ldentification No. 0100148
Dear Mr. Gourley:
The Closure Plan for the above-referenced facility, received by the Bear River Health
Department on February 23, 2OOO has been approved subject to the noted modifications,
if any. This department and the local fire department must be notified 72 hours before
beginnlng closure activities. The Health Department does charge inspection fees (see
enclosures).
The Closure Plan approval is effective for a period of one year from the date of approval.
lf closure does not take place within one year, submittal of a new Closure Plan will be
required, unless otherwise approved by the Executive Secretary.
Our records indicate that the tank remover you intend to use has a certificate which expires
within 5 months. Please ensure that your certified remover has a valid certificate during
closure activities.

Any change to the approved closure Plan must be submitted in writing and approved by
oui office-before implementation. lf contamination is suspected or found during closure
activities, you must report it to the Division of Environmental Response and Remediation
(DERR) at (801 )536-4100 within 24 hours of discovery.
Enclosed is a copy of the Closure form which must be completed and submitted to
the Executive Secretary (UST)Please submit the environmental and Unified Soil
classification (USC) sample analysis data and Chain of Custody forms with the Closure
Notice as soon as possible, but no later than 90 days after tank closure.

inlormation you have submitted. The inspection report indicates that the leak detection method you are using
on this tank is "tank testing and inventory control," and the ATG is used only to provide the prodr.rct level
readings for the inventory reconciliation. As I indicated in my letter of 515199, if yor-r want ro use tank testing
and inventory control as your leak detection method fbr this rank, you can only use it for approximately another
2 years. If you do use this method, you must perform the monthly reconciliation of the final nronthly over/short
value to the allowable of lo/c of the tank'.s throughput plus I 30 gallons, and keep records of that reconciliation.
Il you want to use Automatic Tank Gauging as your leak detection method, you must perform at least one valid
leak test each month, and keep the results of those tests. The results you sent are all right for April, but would
not be for March, because there was not a valid test run during the month (if all the tests thar were run on the
tank for March are shown on the printout you sent). Whichever method you use, please be sr-rre the method is

The Utah Underground Storage Tank (UST) Act provides that the Executive Secretary (UST) may revoke a
Certificate of Compliance if it is determined that a facility is not in substantial compliance with all state and federal
UST statutes, rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and Remediation (DERR)
performed a compliance review for the underground storage tanks at the above facility. The compliance review,
performed on April 22,1999, indicates you have not satisfied the requirements of the Act and are out of compliance.
To achieve compliance you must do the following:
L Submit evidence that proper lnventory Control is performed on UST # I (12,000 gallon gasoline) by;
a. Documenting that inventory records are reconciled monthly to determine the total overage or shortage for
each UST, and the variance is compared to the allowable of 130 gallons plus one percent of the UST's
monthly throughput. The inspection report indicates that the inventory records are being kept, but the
reconciliation is not being done at the end of the month. Please submit records for the last l2 months to show
that the reconciliation is done.
b. Documenting that you meter the product withdrawals and maintain the dispensers to a calibration accuracy
of six cubic inches for every five gallons of product withdrawn.
Other: The inspection report indicates that the primary leak detection method used for this tank is "Tank Tightness
Testing and Inventory Control," with the Automatic Tank Gauge (ATG) being used only to provide the stick
readings for the inventory control. The Federal UST regulations require that, once an UST is upgraded, Tank
Tightness Testing and Inventory Control can be used as the leak detection method for only 10 additional
years after the upgrade. Information we have on file indicates that the tank was upgraded in 1991; if this
is the case you would have to cease using tank testing/inventory control by 2001 and begin using the monthly
ATG leak tests as the leak detection method on the tanks.
The above information must be submitted to this office by June 15, 1999. If it is not received by the date specified,
we will commence the revocation process of your Certificate of Compliance. If you have questions concerning this

The Utah Underground Storage Tank Act (Act), Utah Code Ann. $ 19-6-414 provides that the
Executive Secretary (UST) may revoke a Certificate of Compliance if it is determined that the requirements
specified in Utah Code Ann. $$ 19-6-412(2),4t3 are not current. Utah Code Ann. g l9-6-412(2)(c)
requires substantial compliance with all state and federal statutes, rules, and regulations including Utah
Admin. Code R3l l-202-1, which incorporates by reference Code of Federal Regulations (40 CFR) $ 280.
At the request of the Executive Secretary (UST), the Division of Environmental Response and
Remediation ("DERR") performed an inspection of your facility, through contracting with Bear River
Health Deparfrnent and a compliance review for the Certificate of Compliance issued to a facility owned
or operated by Questar Gas Company ("you"). This facility is located at 45 East 200 North, Logan, Utah.
The designated facility identification number is #0100148. This facility contains one (1) Underground
Storage Tank ("UST").
The compliance review indicates you have not satisfied the requirements of the Certificate of
Compliance. If true, you are violating the eligibility requirements for the Petroleum Storage Tank (PST)
Fund and your Certificate of Compliance may be revoked. To achieve compliance you must do all of the
following:
(l)Submit a copy of a precision line tightness test for the lines associated with tank #1 performed in
accordance with the requirements of the Act, utah Admin. code, R311-202-1, and 40 cFR $$
280.41 (b) and 280.44(b).
Submit a copy of an annual test for proper operation of the automatic line leak detectors on the
pressurized lines associated with tank #1 performed in accordance with the requirements of the
Act, Utah Admin. code, R311-202-1, and 40 CFR $ 290.44(a).

(3)The records establish you have steel USTs that have cathodic protection. Document that you have
had a qualified cathodic protection tester test your cathodic protection system on the tank and
product piping within the last three years as required by Utah Admin. Code R31I-202-l and 40
cFR $ 280.31(b).
(4)Submit evidence that you are properly using your Automatic Tank Gauging (ATG) System, as
required by Utah Admin. Code, R31l-202-1 and 40 CFR $ 280.43(d), by:
(a) Documenting that the ATG System performs and records a leak test at least once a month.
(s)Submit evidence that UST records are maintained in accordance with Utah Admin. Code, R311-
202-l and 40 CFR $$ 280.34 and 280.45 by:
(a)Documenting that the results of monthly ATG Leak Tests are maintained and available for
the last l2 months.
You have 60 days following the receipt of this letter to demonstrate compliance with the requirements of
the Act or your Certificate of Compliance will be revoked and you will be ineligible to receive deliveries
of fuel and receive payment for claims against the PST Fund.
If you fail to resolve the compliance problems described above within 60 days of receipt of this
Ietter, an Order Revoking the Certificate of Compliance will be issued to you.

The Division of Environmental Response and Remediation (DERR) records show you have one or more underground storage
tanks (USTs) which have not met the December 22, 1998 UST upgrade requirements. These tanks are located at QUESTAR
GAS, 45 E 200 N, LOGAN, facility ID# 0100148.
Under Federal regulations issued in 1988, UST owners and operators have until December 22, 1998 to upgrade, replace, or
permanently close USTs that do not meet the requirements for protection against spills, overfills, and corrosion.
This deadline will not be extended. If your tanks are not in compliance by the deadline, they will not be able to receive
deliveries of fuel.

Message: Last January a compliance inspection was performed for Questar's underground storage tank at 45
East 200 North, Logan, Utah (facility 0100148). A compliance letter was sent February 10, 1998, outlining
several items which needed to be addressed. A copy of the letter is enclosed. To date, no response to the
letter has been received. Please submit the requested items as soon as possible. Note item #3, concerning
cathodic protection of the tank and piping. There was some uncertainty as to whether they are cathodically
protected. Iftheyarenot,theUSTmustberemovedorupgradedbyDecemberZZ,l99S; iftheyareprotected,
the cathodic protection must be tested every three years. If the tank and piping are protected, please submit

The Utah Underground Storage Tank (UST) Act provides that the Executive Secretary ruSD may revoke a Certificate
of Compliance if it is determined that a facility is not in substantial compliance with all state and federal UST statutes,
rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and Remediation (DERR)
performed a compliance review for the underground storage tanks at the above facility. The compliance review,
performed on January 9. 1998, indicates you have not satisfied the requirements of the Act and are out of compliance.
To achieve compliance you must do the following:
2. Submit a copy of a current annual (performed in the last 12 months) test for proper operation of the automatic line
leak detector on the pressurized product line associated with UST # I .
3. Submit documentation to show whether the tank and piping are cathodically protected. The inspection report
indicates the tank and piping are not cathodically protected. An inspection done in 1995 indicated the tank and
piping were protected and reported a passing test result of a cathodic protection test done in 1994. If the tank and
piping are cathodically protected, a test of the cathodic protection system must be done every three years. If the
tank and piping are not protected, they must be protected or removed by the December 1998 upgrade deadline.
If you are performing Automatic Tank Gauging as your leak detection method on UST #1, meet item 4 below. If you
are doing Tank Tightness Testing/Inventory Control as your leak detection method, meet item 5 below:
4. Submit evidence that proper Automatic Tank Gauging (ATG) is performed on UST # 1 by
a. Documenting that the ATG system performs a leak test at least once a month. Please submit copies of records
to show that the monthly leak tests have been performed for the last 12 months.
5. Submit evidence that proper Tank Tightness Testing/Inventory Control is performed on UST # 1 by:
a.Submitting a copy of a current (performed in the last l2 months) precision tank tightness test for UST # I .
l. Submit a copy of a current (performed in the last 12 months) precision line tightness test for the product line
associated with UST # 1 (12.000 gallon premium).

b. Submitting copies of properly-completed inventory control records for the last 12 months. The inspection
report indicates the records were not on site for the inspection.
c. Documenting that you meter the product withdrawals and maintain the dispensers to a calibration accuracy of
six cubic inches for every five gallons of product withdrawn.
The above information must be submiued to this office by March 15, 1998. If it is not received by the date specified,
we will commence the revocation process of your Certificate of Compliance. If you have questions concerning this

The Utah Underground Storage Tank (UST) Act provides that the Executive Secretary OST) may revoke a
Certificate of Compliance if it is determined that a facility is not in substantial compliance with all state and
federal UST statutes, rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and Remediation
(DERR) performed a compliance review for the underground storage tanls at the above facility. The compliance
review, performed on November 22, 1995, indicates you have not satisfied the requirements of the Act and are
out of compliance. To achieve compliance you must do the following:
!'', l. Submit a copy of a current precision line tightness test for the producf lines associated with tank
#1.
,-1'', 2. Submit a copy of a current aalrral test for proper operation of the automatic line leak detectors on
the pressurized product lines associated with tanl$ #1.
D'i(1, Submit evidence that proper Automatic Tank Gauging (ATG) is performed on USTs #lby:
Documenting that the ATG system performs and records a valid leak test at least once a
month.
Documenting that the results of monthly leak tests are maintained and available for at least
the last 12 months.
Please return a copy of this speed memo with the above items to speed processing. The above information must
be submitted to this office by February 26,1996. If it is not received by the date specified, we will commence
the revocation process of your Certificate of Compliance. If you have questions concerning this matter or need
