There is one Underground Storage Tank (herein referred to as "UST") located at the facility, consisting of one steel UST used to store and dispense a petroleum solvent. The UST has been registered with the Division of Environmental Response and Remediation ("DERR") at rhe above site and has been assigned facility identification number 6000016. Records maintained by the DERR indicate that a Certificate of Compliance has never been issued to the facility as required by Utah Code Ann. $ l9-6-412(1)b. EXECUTIVE SUMMARY One So0-gallon used oil underyround storage tank was removed for permanent closure ftom the Bettridge Distributing Main office in Cedar City, Utah on May 13, 1993. Oil-stained soilwas observed below the fill spout on the east end of the excavation. Following the collection of UST closure samples, approximately 30 yards of oil-affected soil was removed from the excavation to a point where no evidence of oil impact was observed. The removed soil was disposed at the lron county Landfill. Five confirmatory soil samples were collected from the enla€ed excavation to evaluate the concentration of oil remaining in thesoil. Laboratory analyses of confirmatory soil samples indicated that one of the five samples contained total recoverable hydrocarbons (420 ppm) in excess of Utah Recommended Cleanup Levels for sites with a Level I (100 ppm) or Level ll (300 ppm) environmental sensitivity. The referenced site would most tikely be classffied as a Level I or Level ll site. CLOSURE ASSESSMENT 1.1 Underground Storage Tank Removal One 5oo-gallon used oil underground storage tank was removed for permanent closure on May 13, 1993 from the Ken Bettridge Distributing Main Office property (Facility ldentification No. 6000016) tocated at 386 North 100 West in Cedar City, Utah. Wasatch Environmental, lnc. provided both the tank handling and closure sampling services. The underground storage tank was located on the west side of the facility building; the area immediately surrounding the tank was covered with asphalt pavement. lmmediately prior to removal, the tank atmosphere was 2% of the lower explosive level and the orygen content was 19ol0. Upon removal, the tank was found to be in poor condition. A 7-inch section of a seam on the east end of the tank was conoded; perforations were observed in the tank bottom. Subsequent to tank removal, the residual sludge was removed and the tank was triple rinsed. The sludge remains on site in a 2S,gallon drum pending disposal; rinseate was hauled to Advanced petroleum Recycling tor disposal. The removed tank was hauled to Dixie Disposal in st. Geo.ge, utah. 1.2 Closure Assessment A closure assessment was performed at the site coincident with tank removal on March.l3, 1gg3. The closure assessment included: . A visual, olfactory, and geologic inspection of the excavation; . Screening of the excavation for volatile organic vapors with an organic vapor monitor; . Collection of soil samples for laboratory analyses; . Classification of native soil according to the Unified Soil Classmcation system. The tank was buried under approximately 20 inches of fill. Afler tank removal, the tank excavation measured approximately six feet wide by eight feet long by six feet deep. Native soil exposed in the walls of the excavation crnsisted o, gravelly sand (sp) as shoivn on the soil Analysis Report in appendlx A. Groundwater was not encountered in the excavation. Petroleum stained soil was obsorved around the tank's fill pipe; the fill pip6 was located at the east end o, the tank. 1.2.1 Organic Vapors The east and west ends of the excavalion were screened with an organic vapor monitor equipped with a photoionization detector. This instrument is calibrated to a 1oo ppm isobutyleno standard, "nj h"" "reporting range of 0.1 to 2,000 ppm. An organic vapor concentration of 1o.s ppm was measured at six feet below grade from the east end of the excavation. An organic vapor concenlration of 3.5 ppm was measured at six feet below grade from the west end of the excavation. A suspected release was reported to the Division of Environmental Response and Remediation based on observed soil staining and the elevated organic vapor measurements. The release code given by the Division to the site is EICE. 1.2.2 Sample Colloction and Laboratory Analysis Underground storage tank environmental closure samples were collected from the excavation by a state- certified groundwater and soil sampler (GS 0422) in accordance with site assessment protocol as outlined in state Rule R31'l-205-2. One soil sample (SS1) was collected in native soil at approximately six feet below grade ftom the westem end of the tank excavaton. A second soil sample (SS2) was collected ftom approximately six ,eet below grade from the eastem end of the tank excavation. soil sample locations are shown in figure 1. The soil samplqs were collec{ed in 4-ounce wide-mouth clear glass jars with TEFLON-lined lids and placed into a cooler containing ice immediately following sample mllection and subsequenfly transported under chain of custody to a utah-certified laboratory for analysis of total recoverable petroleum hydrocarbons according to EPA Method 418.1, benzene, toluene, ethylbenzene, total rylenes, and naphthalene according to EPA Method 8020, and halogenated organic compounds according to EpA Method 8010. 1 .2.3 Laboratory Results The soil sample collected ftom the westem end of the tank excavation (SS1) contained 13O,OOO ppm totat recoverable petroleum hydrocarbons; no aromatic or halogenated organic compounds were detected. The soil sample collected ftom the eastem end of the tank excavation (SS2) contained 24,000 ppm totat recoverable petroleum hydrocarbons, 1.000 ppm total xylenes, and 0.g70 ppm naphthalene. No halogenated organic compounds were detected. copies of the laboratory reports from the underground storage tank closure are presenred with a completed Closure Notice in appendix A. Wasatch Environmental, lnc. 2 Results of laboratory chemical analysis indicated the soil collected from the both ends of lhe excavation contained total recoverable petroleum hydrocarbon concentrations above Utah recommended cleanup levels. 2. CORRECTIVE ACTION 2.1 Soil Removal and Confirmatory Soil Sampling On May 13, 1993, the excavation was enlarged to remove oil-affected soil. Following soil removal the excavation measured approximately 9 feet wide by 12leet long by 8 feet deep. Approximately 30 yards of removed soilwas hauled to the lron County Landfillfor disposal; Mr. Jerry Jorgenson, Environmental Health Scientist of the Southwest Utah Public Health Department approved and coordinated the soil disposal. Soil sample CSS3, collected from the west wall of the excavation at a depth of eight feet, contained 26 ppm total recoverable petroleum hydrocarbons. Soil sample CSS4, collected from the south wall of the excavation at depth of eight feet below grade, contained 14 ppm total recrverable petroleum hydrocarbons. Soil sample CSSs, collected from the east wall of the excavation at a depth of eight feet below grade, contained 44 ppm total recoverable petroleum hydrocarbons. Soil sample CSS6, collected from the north wall of the excavation at a depth of eight feet below grade, did not contain a detectable concentration of total recoverable petroleum hydrocarbons. Soil sample CSS7, collected from the central bottom of the excavation at a depth of nine feet below grade, contained 420 ppm total recoverable petroleum hydrocarbons. Copies of the laboratory reports ftom confirmatory soil sampling are presented in appendix B Wasatch Environmental, lnc. 3 1.3 Closure Assessment Summary Following soil removal, five confirmatory soil samples were collected ftom the four walls and the centEl bottom of the excavation. The soilwas placed in +ounce glass iars with TEFLONlined lids, put immediately into an iced cooler, and subsequently transported under chain of custody to a Utah-certified laboratory for analysis of total recoverable petroleum hydrocarbons (EPA Method 418.1). Confirmatory soil sample locations are shown in figure 2. 2.2 Confirmatory Soil Sampling Results 2.3 Summary of Corrective Action ApProimately 30 cubic yards of used oil afiected soil was rcmoved from an excavation that formerly contained one 500 gallon waste oil underground storage tank at the Bettridge Main Offce in C€dar City, utah. The removed soil was hauled to the lron county Landfill for disposal. Laboratory analyses of confirmatory soil samples indicates that the majority of oil affected soil has been removed. The total recoveEble petroleum hydrocarbon mncentration ftom soil sample cssT exceeds the utah Recommended Cleanup Level for sites with Lev€l I and Level ll Environmental Sensitivity, but does not exceed the Utah Recommended Cleanup Level for sites with Level lll Environmental Sensitivity. Submit an application for the PST Fund and obtain a Certificate of Compliance for the faciliry identified above, containing tanks #1 and #2. The Act gives the Executive Secretary (UST) of the Utah Solid and Hazardous Waste Control Board (Board) the power to enforce rules made by the Board by issuing Notices of Agency Action or Orders. The Act, Utah Code Arm., Sections 19-6-407 and 19-6408, also gives the Executive Secretary QST) the power to assess a civil penalty of $1,000 if an owner or operator of an UST fails to register the tark. $1,000 for failure to pay fees within 60 days of due date, and assess interest on the unpaid balance. Other violations of the Act may result in civil penalties of up to $10,000 per day. This letter will advise you that your failure to compty with the requirements of the statutes and rules may result in the issuance of a Notice of Agency Action or Order and the assessment of a civil penalty.
