The Division of Environmental Response and Remediation (DERR) records show you have one or more underground storage
tanks (USTs) which have not met the Decembe r 22, 1998 UST upgrade requirements. These tanks are located at HILLS AND
HOLLOWS SERVICE, P O BOX I4OO
Under Federal regulations issued in 1988, UST owners and operators have until December 22, 1998 to upgrade, replace, or
permanently close USTs that do not meet the requirements for protection against spills, overfills, and corrosion.
This deadline will not be extended. If your tanks are not in compliance by the deadline, they will not be able to receive
deliveries of fuel.
Our information shows these requirements have not yet been met:

Any metal flex connectors connected to fiberglass reinforced plastic (FRP) piping at the submersible pump or dispenser must be

prevention requirements as part of the upgrade requirement.

Dear Mr. Owen:
The Closure Plan for the above-referenced facility, received by the Division of
Environmental Response and Remediation (DERR) on October 22,1998, has been approved subject
to the noted modifications, if any. The DERR office and the local fire department must be notified
72 hours before beginning closure activities. Local agencies may have additional requirements for
closure or may charge inspection fees.
The Closure Plan approvai is effective for a period of one year from the date of approval.
If closure does not take place within one year, submittal of a new Closure Plan will be required,
unless otherwise approved by the Executive Secretary (UST).
Any change to the approved Closure Plan must be submitted, in writing, and approved by the
Executive Secretary (UST) before implementation. If contamination is suspected or found during
closure activities, you must report it to the DERR at (801) 536-4100 within 24 hours of discovery.
Enclosed is a copy of the Closure Notice form, which must be completed and submitted to
the Executive Secretary (UST) after the closure is performed. Please submit the Closure Notice as
soon as possible, but no later than 90 days after tank closure.
If during upcoming closure activities, it appears that a confirmed petroleum release has
occurred, then the conditions, as explained in the attached notice, may apply to your site, provided
the facility complies with all of the applicable rules and regulations.

Division of Environmental Response and Remediation (DERR) records show you have one or
(USTs) which have not met the December 22, 1998 UST upgrade requirements. These tanks

Under Federal regulations issued in 1988, UST owners and operators have until December 22, 1
permanently close USTs that do not meet the requirements for protection against spills,
This deadtine will not be extended. If your tanks are not in compliance by the deadline,
deliveries of fuel.
The DERR encourages owners and operators to meet the 1998 upgrade requirements as soon as
1998 deadline. It can take several months to upgrade, close or replace an UST system. Delays
may not be available if you wait. Start planning now!
Our information shows these requirements have not yet been met:

Any metal flex connectors connected to fiberglass reinforced plastic (FRP) piping at the

(2) Documenting that inventory records are reconciled monthly to determine the total overage
or shortage for each UST, and the variance is compared to the allowable of 130 gallons
plus one percent of the UST's monthly throughput.
(3) Documenting that the water in each UST is measured to within one-eighth inch, at least
once per month, and the water measurements, converted to gallons, are used to corre0 the
monthly inventory reconciliations.
The above information must be submitted to this offlrce by lll5l97. If it is not received by the date
specified, we will commence the revocation proces of your Certificate of Compliance. If you have

To continue to maintain a valid Certificate of Compliance, the facility must be in compliance with Federal
and State Rules and Regulations. An inspection of your facility and a review of our records has shown
that the following information is lacking:
tl I Submit evidence that proper Inventory Control is performed on USTs_!. an!.L , by;
(a ) Documenting that inventory records are reconciled monthly to determine the total overage
or shortage for each UST, and the variance is compared to the allowable of 130 gallons
plus one percent of the USTs monthly throughput.
(b ) Documenting that drop tubes extending to within one foot of the tank bottom have been
installed in USTs 1. and 2 .
(c ) Documenting that you have obtained a gauge stick that is capable of measuring product
level in one-eighth inch increments, is legible, is long enough to reach the bottom of the
tank, and is not wom on the end.
(d ) Documenting that the water in each UST is measured to within one-eighth inch, at least
once per month, and the water measurements, converted to gallons, are used to correct the
monthly inventory reconciliations.
Submit 2 months of information for items la, and ld. A booklet describing inventory control
procedures in enclosed.
The above information must be submitted to this office by 7ll5l95.If it is nor received by the date
specified, we will commence the revocation process of your Certificate of Compliance. If you have

Dear Mr. Lyman:
The Closure Plan for the above-referenced facility, received by the
Bureau of Environmental Response and Remediation on February 25, L99l,
has been approved subject to the noted modifications, if any. Local
health and fire departments must be notified 72 hours before beginning
closure activities. These agencies may have additional requirements for
closure or may charge inspection fees. The Bureau of Environmental
Response and Remediation must also be notified 72 hours before closure
begins.
Any proposed change to an approved Closure PIan must be submitted in
writing and approved by the office of the Executive Secretary before
implementation. If contamination is suspected or found during closure
activities, you must report it to the Bureau of Environmental Response
and Remediation within 24 hours of discovery.
Enclosed is a copy of the lrClosure Noticett form which must be completed
and submitted to the Executive Secretary (UST). The closed tank records
may then be removed from the fee billing lists. Please submit Sample
Analysis data and Chain of Custody forms with the Closure Notice as soon
as possible, but no later than 90 days after tank closure.

summer, when it will- be used to rnake asphalt. It will be mixed

The closure plan, received by the Bureau of Environmental Response and Remediation, on August(r, i990 for the above referenced facility has been approved ai proposed. Local health and fire

tanks referred to in the closure plan may commence after notifying these agencies-.
Enclosed is a copy of the Closure Notice form which must be received by the Executive Secretary
before the closed tanks may be removed from the fee billin-e list. Pleas" titt out all the requested

other information is available.
