SITE NAME: Great Basin Trucking Lot 2
SITE LOCATION: West Valley City
DATE OF SITE VISIT: 21 November 2025
PURPOSE OF SITE VISIT: DERR (David Bird) oversight of Annual Inspection at the site.
FIELD OBSERVATIONS: On 21 NOV at ~12 pm, the DERR met with AGEC Consulting for the Annual
Inspection pursuant to the Site Management Plan (SMP) for the site. The DERR and AGEC walked the
site and followed the checklist included with the SMP. All items inspected were acceptable. AGEC
indicated they would send an inspection report to the DERR.
The DERR left the site at ~12:20 pm.
