Jake has cleared out the deep rooted vegetation and the roots in the spring collection pipes. See the below pictures for
documentation. Please let us know if you need anything else to resolve these deficiencies and close this CAP.

Good Afternoon,

We received this extension to the Corrective Action Plan (CAP) for the Piuta Mia Camp. We now have until the end of this
year to remove the deep-rooted vegetation in the spring collection area. This will need to be documented (a photo is
suggested) and submitted to Utah DDW. Please let us know when this has been taken care of and if you need any further
assistance.
