The Division of Environmental Response and Remediation (DERR) received the Work Plan
and Budget (WP&B) and Work Plan Approval Application and Agreement on February 20, 2024.
The WP&B dated February 12, 2024 was prepared by Underwood Environmental Consulting, Inc.
(UEC). This WP&B is for two rounds of semi-annual groundwater monitoring for nine onsite
monitoring wells (MW-2, MW-3, MW-4, MW-9, MW-10, MW-11, MW-35, MW-36, and MW-37)
at the above-referenced site.
The DERR approves the WP&B up to $10,509.16. This approval is contingent that the costs
being submitted for reimbursement meet the requirements of the rule changes that became effective
on September 13, 2021, including the incorporation of the Cost Guidelines for Utah Underground
Storage Tank Sites document. A copy of UEC’s budget spreadsheet is attached. Please submit the
first Semi-Annual Groundwater Monitoring report documenting this work by May 20, 2024. The next
Semi-Annual Groundwater Monitoring report should follow in six months for a total of two reports.
Requests for Petroleum Storage Tank (PST) reimbursement must be submitted in accordance
with Utah PST Rule R311-207-4 and R311-207-5. Please reference WP&B No. NYL-04 on your
Request for Reimbursement. Payment for work conducted under this WP&B will be for actual time
and materials expended up to, but not exceeding, the amount of the approved line-item budget.
Payment shall be limited to documented costs that are customary, reasonable, legitimate, and directly
related to the work tasks performed. Requests for payment must be received by the DERR within one
year of the date that the work was performed or reimbursement shall be denied in accordance with
PST Rule R311-207-3(8).
