This Approval expires on DATE. (December 31, 20XX, ≥ 5 yrs from the date of this letter)
Dear System Contact:
The Division of Drinking Water (the Division) received the following documents on DATE:
The 20XX water line installation drawings and specifications for WATER SYSTEM NAME, stamped and signed by NAME, P.E., and dated DATE.
The completed application form signed by NAME and dated DATE, which formally designates:
NAME, P.E., as the Professional Engineer (PE) having direct responsibility for the design of the entire water, and
NAME, P.E., as the PE responsible for oversight of the hydraulic analysis of the entire water system.
We completed our review of the 20XX water line installation drawings and specifications, and find they basically comply with the applicable portions of Utah's Rules for Public Drinking
Water Systems in R309-550. On this basis, WATER SYSTEM NAME 20XX water line installation drawings and specifications are hereby approved.
WATER SYSTEM NAME has met the criteria in R309-500-7 to install water lines up to 16 inches in diameter and is not required to obtain Plan Approval or an Operating Permit for the qualified
water line projects.
Please note the following restrictions to this approval:
1. This approval does not apply to booster pump stations.
2. If minimum separation between water and sewer lines cannot meet the requirements in R309-550-8, an exception must be obtained prior to water line construction.
This Approval expires on DATE (June 30, 20XX, ≥ 5 years from the date of this letter if this letter is generated between January and June; December 31, 20XX, ≥ 5 years from the date
of this letter if this letter is generated after July 1). You must notify the Division if there is a change to standard installation drawings and specifications or the designated PE
for design or hydraulic analysis during the term of this Approval. To continue using standard installation drawings and specifications for water transmission and distribution lines after
the expiration date, you must submit a complete renewal application to the Division.
Please maintain a copy of this letter with your permanent records for future reference.
