Dear Keith Fryer:
The Division of Drinking Water (the Division) received a request for an Operating Permit for Bridlewalk Acres Phase 4 water lines from you on February 22, 2024. The following is a summary
of the Division’s review of this project:
The Division received a project notification form (PNF) and plans and specifications for the project on August 22, 2022.
Division staff provided review comments to Oquirrh Mountain Water Company on August 31, 2022. No revisions were received in response to the review comments.
The Division received an Operating Permit request and as-built plans for Bridlewalk Acres Phase 4 on February 23, 2024. The Division had not received plans or issued plan approval for
the project before construction was completed.
The Division received responses to the review comments on March 8, 2024, as well as a hydraulic modeling certification letter signed by Aaron Anderson, P.E., with Hansen, Allen, & Luce.
The project was the installation of approximately 4,384 linear feet of 8-inch diameter PVC (C900) drinking water line with associated valves, fittings, and appurtenances. The project
included nine (9) new fire hydrants and thirty-seven (37) new service connections.
We have received the following information for Bridlewalk Acres Phase 4 water lines:
Certification of Rule Conformance by a professional engineer including a statement that no changes were made during construction.
As-built drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Bridlewalk Acres Phase 4 Water Linesis hereby issued as constituted
by this letter. You may now place Bridlewalk Acres Phase 3 Water Lines in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
