The Division issued Plan Approval for this project on March 13, 2024. Division staff conducted a final inspection as part of the Sanitary Survey on May 12, 2025. Revised As-built drawings were submitted to the Division on July 9, 2025. Our understanding of the project was the construction of a new 25,000-gallon buried concrete water tank (ST003) with a new pump house (PF002) constructed on top of the tank that will provide pressure to the distribution system. These facilities will replace the existing above-ground water tanks (ST001 and ST002) and the existing pump station (PF001). We have received the following information for Cistern Tank (ST003) and Pump House (PF002): Certification of Rule Conformance with plan approval conditions by a professional engineer and changes made during construction were in conformance with rules R309-500 through 550. As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Cistern Tank (ST003) and Pump House (PF002) is hereby issued as constituted by this letter. You may now place Cistern Tank (ST003) and Pump House (PF002) in service in your water system. Please maintain a copy of this letter with your permanent records for future reference. If you have any questions regarding this Operating Permit, please contact Curtis Page, P.E., of this office, at (435) 650-7795, or Michael Newberry, P.E., Permitting and Engineering Manager, at (385) 515-1464.
