Dear LeRoy Daines: Subject: RTCR Level 2 Assessment for Cove Fort Service Inc Water System, Utah #14066 Thank you for taking the time to on September 30th to discuss the bacteriological sample history of the Cove Fort Service Inc water system. The history of failing bacteriological samples has initiated the need to conduct a Level 2 Assessment (L2) of the Cove Fort Service Inc water system. The L2 was required due to the water system having two level 1 assessments within a 12-month period. Observations:
The Cove Fort Service Inc water system has had a recurring issue of failed bacteriological samples during the late summer months and into the Fall season for the past few years. I have personally worked with previous owners of the store to disinfect and flush the well during the late summer months due to the well producing total
coliform positive water samples. The timing of the failed water samples coincides with the warmest times of the year, and the temperature of the water from the well is relatively warm to begin with.
Previous evaluations of this water system recommended that the in-line cartridge filter located in the well discharge piping be replaced more frequently to reduce the potential for bacteria to begin growing within that filter and it is my understanding that that was done successfully for a while. However, subsequent source sampling showed that samples taken directly from the well were total coliform positive. It should also be noted that with the most recent change of ownership of the store, the well discharge piping was reconfigured and a large pressure tank was replaced with two smaller ones and the in-line cartridge filter was removed. Even after these upgrades, the system still had failing bacteriological samples in July and August of this year. The monthly routine sample, along with the repeat and triggered source samples, all tested positive for total coliform in those two months.
Conclusion: It appears that each summer when outside temperatures rise and the well water temperature raises also, it is
allowing for bacteria growth in the well water. Annual maintenance to disinfect and flush the well may help, but with the problem occurring in multiple months, it won’t be as effective as needed.

In our discussion about the problem, you indicated that the Owner of the water system is working to hire a
consulting engineer to design a chlorination system for the Cove Fort Service Inc water system. I agree that a permanent, continuous disinfection system is the best option to maintain clean drinking water for your water system.
Recommendations:
1. Contract with a consulting engineer to assist you with designing and permitting a new continuous disinfection system for the Cove Fort Service Inc water system. 2.
