UTAH26023 Water system violation--Storm Haven

The attached document was sent to the customers of UTAH 26023 in response to the water system violation noted in 2 separate letters dated
July 15, 2025 for the Storm Haven water system in Daniel, Utah. The violations were that a sample from the well had not been collected for the
bacteriological analysis, though samples had been taken elsewhere in the system, and the results were that coliform was not present. This was
a new operator who did not realize that we are now required to collect 2 samples instead of 1 each month, and this occurred in the months of
April and May 2025. We are now aware and are collecting the 2 samples each month beginning with June.
