Dear Mr. Bird,
In accordance with condition I.S.2.c of Clean Harbors Aragonite's (CHA) Title V Permit, this
correspondence serves as written notification that the referenced facility will be retracting its
submiual of a permit deviation from condition II.B.3.b.3 by not reporting the Semi-Annual 40
CFR 6l Subpart V and 40 CFR 63 Subpart DD Reports.
On April 9, 2024, per correspondence with the Senior Environmental Compliance Manager of
Clean Harbors Aragonite, LLC Mr. Kenneth Banks, and the UDAQ inspector Mr. Joe Rockwell,
there was an error in submitting the deviation report on April 8,2024. Per condition II.B.3.b.3,
there is not a 30-day requirement after the reporting period to submit the Semi-Annual 40 CFR
6l Subpart V and 40 CFR 63 Subpart DD Report. This deviation report was submitted in error,
and CHA is requesting to retract the deviation report submiual.
In accordance with Permit Condition I.K and Utah Administrative Code R307-415-5d, I certifu
under penalty of law that this document and all attachments were prepared under my direct
supervision in accordance with a system designed to assure that qualified personnel properly
gather and evaluate the information submitted. Based upon my inquiry of persons or persons who
manage the system, or those persons directly responsible for gathering the information submitted
is, to the best of my knowledge and belief, true, accurate and complete. I am aware that there are
significant penalties for submiuing false information, including the possibility of imprisonment
for known violations.
