Kamlowsky, Brown Consulting Engineers, P.C., on September 17, 2025. The Division received the plans and specifications on May 17, 2024, and written review comments were provided to you and your consultant on May 17, 2024. The Division received the revised plans and specifications on June 3, 2024. The Division issued Plan Approval for this project on June 6, 2024. Our understanding of the project was to extend a main water line from the corner of Mountain View Drive, east along the Pine Valley Highway, to provide water service to a new church that was built. The water line extension is approximately 3,400 linear feet of 8-inch diameter PVC C-900 pipe, including fire hydrants, isolation valves, and appurtenant fittings. We have received the following information for the Pine Valley Highway Water Line Extension: Certification of Rule Conformance with plan approval conditions by a professional engineer. As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Pine Valley Highway Water Line Extensionis hereby issued as constituted by this letter. You may now place this water line in service in your water system. Please maintain a copy of this letter with your permanent records for future reference. If you have any questions regarding this Operating Permit, please contact J.
