Regarding the referenced project, please note the following:

1. The project was completed according to approved plans and conditions, to the best of my
knowledge all construction was completed in conformance with Rules R309-500 through 550.
2. Construction record drawings will be sent with this request and have been provided to Canyon
Meadows Mutual Water Company.
3. Satisfactory bacteriological samples are attached as evidence of proper disinfection and flushing
in accordance with ANSI/AWWA standards for a Water-Storage Facility.
4. The location of the new treatment facility is within the existing pumphouse at
40.40175164252778° N, 111.55119146549961° W.
