I have made changes to the boil order and attached the approved boil order to this email. I did add the September 8th original sample date. We will need to discuss that further as repeat samples are supposed to be taken within 24 hours from notification of the positive sample. Could you send over the original sample information? I put that it was total coliform positive and E. coli absent, is that correct? Can you please update it if that is not correct and send it back to me for approval. Learn why this is important Hello Drew, Just following up to our phone conversation. This is the email chain you can respond with the boil order template and any questions you may have. Per our conversation, we are moving forward with a boil order posted and hand delivered. Attached are the three documents needed for completing a boil order: · Boil Order Example: This is going to help you to write your own boil order. Feel free to work on this document or the Boil Order PN if you would like. · Boil Order PN: This is the template we have for systems to fill out their information and send it back for approval. · Division of Drinking Water Boil Order Instructions: This will provide you with instructions on many things boil order related (including ways to deliver notice). The 3rd page of this document is the certification form that needs to be filled out after you deliver the boil order to the customers. · Certification for Boil Order: This is the above 3rd page just pulled out of that document and created as its own document to submit The Boil Order needs to be filled out and sent back to me for approval. Once it is approved you can move forward with delivering it to the customers. The Boil Order needs to be delivered to the customers within 24 hours. Please send us the boil order template back for approval as soon as possible. We just want to make sure it gives you all time to deliver it to the customers before the 24 hour deadline tomorrow. The triggered source sample was clean, but 2 of the repeat samples were EC+ and the other repeat sample was TC+. This triggers a boil order since we have a confirmed E. coli positive water sample. I have contacted Drew Corry, with the Church of Jesus Christ of Latter-Day Saints, and let him know to post the boil order and do-not-drink on the drinking fountains. Can you let me know, or contact Drew at corryat@churchofjesuschrist.org or 435-994- 3736 to let him know what they need to officially post? I will go down there tomorrow and inspect the system with their staff at Cove Fort. Since the source sample was clean, I suspect that they may have contamination in their storage tank or a cross connection in their system. Thank you for your help with this. John John L.
