Attached is the As-Built Report for the new 2025 Evaporation Pond compliance monitoring wells: GW-158 and GW-159. In accordance with Groundwater Quality Discharge Permit Number UGW450005 (GWQDP) Part II. L., Monitoring Well “As-Built” Reports will be submitted to the Director within 60 days of final completion of each well. GW-158 and GW-159 were completed on June 25, 2025 and slug testing was
performed on June 27, 2025.

Subject: 2025 Evaporation Pond Monitoring Wells Installation
Dear Mr. Schon,
Geosyntec Consultants, Inc (Geosyntec) is pleased to submit this document summarizing the
installation, development, and slug testing of two groundwater monitoring wells near the soon to
be constructed evaporation pond at the EnergySolutions Clive, Utah facility (Site or Facility). Well
installation and well development activities were completed June 23 through 25, 2025. Slug testing
was completed on June 27, 2025. Prior to initiating field work, a Site-specific Task Hazard
Analysis Plan was prepared, site-specific radiation training was taken by the Geosyntec and
Subcontractor personnel, and utility clearance was conducted.
Division of Water Rights Application
In advance of drilling activities, a Non-Production Well Construction application was submitted
to Utah Division of Water Rights (DWR) on May 21, 2025. The application included the planned
locations and proposed dimensions of each monitoring well. The locations of the monitoring wells
are presented on Figure 1. On May 27, 2025, DWR granted approval to proceed with installation
of the two proposed monitoring wells. Following completion of the well installations, the final
signed applicant card was submitted to DWR. Completed applications, forms, and correspondence
from DWR are provided in Attachment A.
Well Installation
Two monitoring wells (GW-158 and GW-159) were drilled and developed in accordance with the
Geosyntec May 19, 2025 proposal. Each well was installed with hollow-stem auger drilling
equipment to total depths of 43 feet below ground surface (BGS). The monitoring wells were
completed with 2-inch PVC casing, 15-foot 0.010-inch screen, 10/20 silica sand filter pack, and
bentonite seal. A 4-foot by 4-foot concrete apron at ground level was constructed around each
well. A 10-inch wide, 3-foot-tall steel protective casing was placed above the apron to protect the
well.
Following installation, Geosyntec personnel developed each well and extracted a minimum of 10
well casing volumes. Additionally, water quality parameters were monitored for stabilization.
Investigation Derived Waste (IDW) was containerized and disposed of by EnergySolutions. Well

construction logs and development data for each well are provided in Attachment B.
EnergySolutions personnel surveyed the locations and elevations of the completed wells.
Slug Testing
Slug testing was conducted in accordance with ASTM D4044-9 to estimate the hydraulic
conductivity in the two wells. Slug testing equipment used at each well consisted of a water level
meter, slug, and transducer. Both rising and falling head tests were conducted at each well.
The transducer data from the slug tests was pre-processed using Microsoft Excel and analyzed
with AQTESOLV, a commercial software program for analysis of slug tests created by
HydroSOLVE of Reston, Virginia. Slug test data was evaluated using the Bouwer-Rice (1976)[1]
straight-line method to estimate hydraulic conductivity. The hydraulic conductivities derived from
the slug test analysis are summarized in Table 1, and AQTESOLV outputs are provided in
Attachment C. Hydraulic conductivity of silty sand can range from 0.1 to 100 feet per day. The
estimated hydraulic conductivities from these slug tests fall within that range.

Regarding your request to drill 7 Monitor well(s), the anticipated drilling depths will exceed the minimum regulated
and reporting depth of 30 feet, thereby requiring permission from the Division of Water Rights to proceed with this
project.
The specifications outlined in your non-production well project application meet the State Engineer's requirements
and permission is HEREBY GRANTED. Therefore, this letter is your authorization to proceed with the
construction of the well(s) in accordance with those specifications and with respect to the following provisions:
1)Small diameter casing is to be used in the construction of the well(s) and no more water is to be diverted than
is necessary to determine the quality of the groundwater by obtaining representative samples as required by
the project.
2)The well(s) must be drilled by a currently licensed Utah driller and must be drilled in a manner consistent with
the construction standards cited in the Utah State Administrative Rules for Well Drillers (R655-4 UAC).
3)The enclosed Driller (START) Card form must be given to the licensed driller for his submittal prior to
commencing well construction. The other enclosed form is the Applicant Card. It is YOUR
RESPONSIBILITY to sign and return this Applicant Card form to our office upon well completion.
4)At such time as the well(s) are no longer utilized to monitor ground water or the intent of the project is
terminated, the well(s) must be permanently abandoned in a manner consistent with the Administrative Rules
(R655-4 UAC).
5)THIS PERMIT MAY NOT BE THE ONLY AUTHORIZATION NEEDED TO DRILL A WELL. The
applicant is responsible for obtaining other permits/authorizations from federal agencies, other state agencies,
and/or local jurisdictions as applicable. Moreover, if the applicant is not the landowner, it is the applicants
responsibility to ensure that approvals/permissions have been obtained to trespass and drill a well(s) on the
property. THIS PERMIT DOES NOT GIVE AUTHORIZATION TO TRESPASS ON PRIVATE
PROPERTY.
