T Allen" <tammieallen@utah.gov>Cc: Jaime Tubbs <jtubbs@centralutahhealth.org> Hi, Sorry I was out last week and am just getting to this. I found results for Cove Fort, Brooklyn, and Central Valley. Central Valley's results were uploaded with CMDP, but somehow didn't gothrough I guess. The other 2 somehow didn't make it into CMDP. All results are attached. Thanks! Was there a Monthly Bac-T for June? The others I have entered! [Quoted text hidden] Mz. I updated our records, made a note and will let Sitara know! [Quoted text hidden] Eric Larsen <elarsen@centralutahhealth.org>Tue, Jul 22, 2025 at 9:23 AM To: "Mz. T Allen" <tammieallen@utah.gov>Cc: Jaime Tubbs <jtubbs@centralutahhealth.org> We figured out what happened with 21006. The system has a new Operator and he submitted the samples under WS001 and WS002, but they should have just been monthly Bac-Ts forDS001. Is there anything we need to do to correct that, or is this email enough? Also, with the Cove Fort Service 14066, there was a routine and a repeat sample that we missed (also figured out what happened). We are thinking of uploading them both to CMDP sothat they're connected--is that the best course of action? Eric Larsen, M.P.H., M.H.A., L.E.H.S.
