INTRODUCTION
1.1 Site Description
Site Name Fleet Management Yard
Site Location/Address 850 South 325 West, Salt Lake City
Land Area Approximately 8.1 acres
General Site Description
The site was formerly used as a Salt Lake City fleet vehicle fueling and
maintenance facility. It is currently used by the Salt Lake City Police
Department as a training facility and the Salt Lake City Fire Department
for equipment storage.
Exhibit 1 (Appendix A) presents the general boundaries and topography of the site on portions
of the Salt Lake City North, Utah 1998 USGS topographic quadrangle map. Exhibit 2 is a site
diagram that displays the area where the work was performed at the site.Exhibit 3 is a site
diagram for the Corrective Action Plan design, Exhibit 4 displays the sampling locations, and
Exhibit 5 displays the extent of excavations conducted at the site.
1.2 Project Background
A fuel release associated with the former fleet fueling system was initially discovered in 1993 at
the site (Exhibit 2). Subsurface investigations and periodic groundwater monitoring were
conducted between 1993 and 2011, and the four underground storage tanks (USTs) were
removed in 2011. In 2013, soil and groundwater impacts were identified at concentrations
above the Utah Department of Environmental Quality’s (UDEQ’s) Division of Environmental
Response and Remediation (DERR) Leaking Underground Storage Tank (LUST) Program
screening levels surrounding the former tank basin. In 2014, Terracon implemented corrective
action under Work Plan IEQ-15 in the vicinity of the former tank basin that included over-
excavation and removal of the identified impacted soils for off-site disposal, removal of impacted
groundwater from the open excavation, and a limited application of chemical oxidation reagents
to treat residual petroleum-impacted groundwater in the excavation area prior to backfilling
(Exhibit 3).
Terracon subsequently installed six groundwater monitoring wells under Work Plan IEQ-16 and
conducted a groundwater sampling event to evaluate concentrations of petroleum hydrocarbons
remaining near the former fuel system. Under Work Plan IEQ-17, four additional borings were

advanced for soil sampling and completed as monitoring wells, and an additional round of post-
corrective action groundwater sampling was conducted. Results from these two groundwater
sampling events indicated that dissolved petroleum hydrocarbons, specifically TPH-DRO and
benzene, remained at concentrations above DERR’s Initial Screening Levels (ISLs) in localized
areas south of the former tank basin.
Based on the results of investigations conducted under Work Plans IEQ-16 through IEQ-18, the
residual petroleum impacts to soil exceeding ISLs appeared to be confined to a relatively small
area within the footprint of an existing building, with dissolved petroleum impacts in the
groundwater exceeding the ISL extending to the southern property boundary.
Salt Lake City Corporation intends to make the site available for mixed-use redevelopment. To
facilitate site redevelopment, Terracon prepared a Corrective Action Plan (CAP; September 6,
2016) intended to reduce residual constituents in soil and groundwater to concentrations below
ISLs to allow for unrestricted land use and receive a “No Further Action” status for Release ID
IEQ. The CAP was approved by DERR on September 15, 2016.
1.3 Scope of Work
The primary objective of Work Plan IEQ-19 was to remove petroleum-impacted soils and
groundwater from an area south of the former UST basin where impacts were identified as
remaining above the ISLs. The selected approach included removal of the impacted soils for
off-site disposal and removal of impacted groundwater from the open excavation, followed by
limited application of chemical oxidation reagents to treat residual petroleum-impacted
groundwater within the excavation area prior to backfilling.
Terracon conducted the site work following the DERR-approved Work Plan IEQ-19. This report
documents implementation of the Work Plan IEQ-19 during January, February, and March
2017.
1.4 Standard of Care
Terracon’s services were performed in a manner consistent with generally accepted practices of
the profession undertaken in similar studies in the same geographical area during the same
time period. Please note that Terracon does not warrant the work of laboratories, regulatory
agencies or other third parties supplying information used in the preparation of the report.
These services were performed in accordance with the scope of work agreed with you, our
client, as reflected in our proposal, and were not restricted by ASTM E1903-11.
1.5 Additional Scope Limitations
Findings, conclusions, and recommendations resulting from these services are based upon
information derived from the on-site activities and other services performed under this scope of
work. Such information is subject to change over time. Certain indicators of the presence of

hazardous substances, petroleum products, or other constituents may have been latent,
inaccessible, unobservable, non-detectable or not present during these services, and we cannot
represent that the site contains no hazardous substances, toxic materials, petroleum products,
or other latent conditions beyond those identified during this investigation and remediation effort.
Subsurface conditions may vary from those encountered at specific borings or wells or during
other surveys, tests, assessments, investigations or exploratory services; the data,
interpretations, findings, and our recommendations are based solely upon data obtained at the
time and within the scope of these services.
1.6 Reliance
This report has been prepared for the exclusive use and reliance of Salt Lake City Corporation
and government entities having jurisdiction over the site. Use or reliance by any other party is
prohibited without the written authorization of Salt Lake City Corporation and Terracon.
Reliance on this report by the client and all authorized parties will be subject to the terms,
conditions and limitations stated in the proposal, report, and Terracon’s Terms and Conditions.
The limitation of liability defined in the Terms and Conditions is the aggregate limit of Terracon’s
liability to the client and all relying parties.
SOIL REMEDIATION
Terracon retained the services of a licensed, insured, and bonded environmental contractor, Direct
Push Services LLC (DPS), to conduct building demolition, soil excavation and remediation
services. All work was conducted under the direction of a Utah UST Consultant, and all
environmental samples were collected by a Utah-Certified Groundwater and Soil Sampler.
2.1 Building Demolition
Prior to implementing soil excavation activities, the scope of work required demolition of
approximately 100 feet of the eastern portion of the southern site building to access impacted
soils beneath the building (Exhibit 3). Terracon worked with the property owner and the
subcontractor to conduct a pre-demolition asbestos and hazardous materials survey and obtain
a demolition permit for the portion of the building overlying the proposed excavation area.
2.2 Soil Excavation Activities
Once the building demolition was complete and the proposed excavation area exposed, soil
excavation was implemented. Excavated soil was field-screened using a photoionization
detector (PID), along with olfactory and visual indications, to assist with defining the extent of
the excavation of impacted soils. An on-site Utah-certified mobile laboratory (Summit Field
Analytical Services) was used to attain real time analytical results to verify concentrations of

petroleum constituents in remaining soils. If the confirmation soil sample analytical results
exceeded an ISL, excavation continued and additional confirmation samples were collected.
The initial scope of work identified the proposed excavation area outlined in Exhibit 3. The CAP
outlined the removal of non-impacted overburden to a depth of approximately 8 to 9 feet below
ground surface (bgs), and the removal of impacted soil from a target depth range of 9 to 11 feet
bgs.
As excavation activities progressed, Terracon collected confirmation soil samples from the walls
and floor of the excavation to document petroleum hydrocarbon concentrations remaining and
assess the need for removal of additional impacted soils to meet cleanup criteria. The samples
were numbered and labeled sequentially with a direction, i.e., “SW” to identify the sample and
area of the excavation from which it was collected. Floor samples are also specified with the
letter “F.” Confirmation samples collected at the boundaries of the proposed extent of
excavation reported concentrations of petroleum hydrocarbons that exceeded the ISLs.
With authorization from the DERR, Terracon directed DPS to extend the excavation boundaries
and continue removing non-impacted overburden and impacted soils from the area. Exhibit 4
shows the final extent of excavated soils and the confirmation soil sample locations.
2.3 Soil Disposal
Approximately 5,017 tons of clean overburden were excavated and removed from the site to
access the impacted soils. Approximately 909.17 tons of petroleum-impacted soils were
transported to E.T. Technologies Soil Regeneration Site (SRS) in Salt Lake City with proper
documentation. A copy of the weight tickets is included in Appendix C.
GROUNDWATER REMEDIATION
3.1 Groundwater Management
Depth to water in the excavation was measured between 9 to 10 feet below ground surface
(bgs) and there was additional infiltration of water from the northern adjoining (2014) excavation
area into the active (2017) excavation area. To manage groundwater during the excavation and
to remove as much petroleum mass from the site, groundwater was extracted during remedial
activities and was stored in four 20,000-gallon holding tanks for treatment by air sparging and
then later chemical oxidation.
Once dewatering activities were complete, water samples were collected from the holding tank
to evaluate the petroleum hydrocarbons concentrations. Initially, the Work Plan called for the
water to be discharged to the nearest on-site storm water collection and discharge point, and an
UPDES construction dewatering permit was acquired from the Utah Division of Water Quality.

hydrocarbons and total dissolved solids (TDS) present at concentrations that exceeded the
permit requirements for discharge.
Following a month of air sparging, three separate applications of activated hydrogen peroxide
treatment were administered to each of the four holding tanks to reduce the dissolved
hydrocarbon concentrations. Follow-up sampling of the water in the tanks reported petroleum
hydrocarbons concentrations were below the ISLs; however, results indicated that the water
would not meet the permit requirements for TDS. Therefore, the treated water was discharged
back into the excavation.
3.2 Vacuum-Enhanced Fluid Recovery
In accordance with the CAP, DPS provided a vacuum trailer to remove impacted groundwater
and free product from within the excavation and existing monitoring wells T-3, T-4, and T-6.
Approximately 250 gallons of dissolved-phase and free product were removed from the
excavation and wells. The water was disposed at VJ Environmental. A copy of the disposal
ticket is included in Appendix C.
3.3 Application of Hydrogen Peroxide
To remediate the residual soil and groundwater contamination in the immediate vicinity of the
excavation, a limited chemical oxidation event was implemented prior to backfilling.
Terracon directed DPS to apply three 55-gallon drums of a 35% solution of hydrogen peroxide
directly into the groundwater at the bottom of the excavation, with the use of a barrel pump, and
then hose-apply the solution to the target areas where residual contamination appeared most
likely to remain. Once the hydrogen peroxide was applied, approximately 25 pounds of ferrous
sulfate was broadcasted into the excavation in an effort to advance the chemical reaction.
ORPHANED UST
During excavation of impacted soils, an approximate 1,000-gallon diesel-containing orphaned
UST was uncovered beneath the former building footprint and within the extent of the
excavation (Exhibit 4). The UST was reported to DERR, and approximately 180 gallons of
product were removed from the UST. The UST was removed after a Closure Plan was
submitted to the DERR and approved to authorize removal, and a follow-up Closure Notice was
subsequently submitted to DERR to document proper removal and sampling protocols. The
UST sampling and removal efforts were separated from the PST-funded efforts, with separate
invoicing to Salt Lake City Corporation to pay for those efforts since the orphaned UST was not
covered by the PST Fund.

SITE RESTORATION
Following the completion of remediation activities, the excavation was backfilled and compacted
in one- to two-foot lifts with a clean backfill material consisting of 1½-inch free-draining gravel
that was placed from the bottom of the excavation to approximately four feet bgs. A geotextile
fabric was placed above the gravel, followed by structural backfill material placed and
compacted as needed to backfill the excavations to existing grade and achieve 95% of a
modified proctor. Based on future plans for site redevelopment, the surficial pavement was not
restored.
MONITORING WELL INSTALLATION
In accordance with the CAP, after the excavation area was backfilled, Terracon returned to the
site to install five groundwater monitoring wells to allow for subsequent groundwater monitoring.
Two of the wells were installed to replace monitoring wells that were located within the
excavation area, and three wells were installed near the downgradient (southern) site boundary.
The installation and sampling of the newly installed monitoring wells will be detailed in a pending
groundwater monitoring event report.
LABORATORY ANALYTICAL METHODS
All confirmation soil samples were collected and placed in laboratory-prepared glassware and
hand-delivered to the on-site mobile laboratory for same day analyses. The samples were
analyzed by laboratory test methods as appropriate for the types of contaminants most likely to
be encountered. This included methyl tertiary butyl ether, benzene, toluene, ethyl benzene,
total xylenes and naphthalene (MBTEXN); total petroleum hydrocarbons – gasoline range
organics (TPH-GRO); and total petroleum hydrocarbons – diesel range organics (TPH-DRO)
using the following methods:

Water samples from the holding tanks were collected in laboratory-supplied containers, placed
on ice in a cooler, and hand-delivered to American West Analytical Laboratory in Salt Lake City.
The UPDES construction dewatering permit required analyses of MBTEXN, TPH-GRO, and
TPH-DRO, lead, oil and grease (O&G), pH, total suspended solids (TSS), and total dissolved
solids (TDS) to document that water held in the tanks met the requirements of the permit prior to

discharge. To evaluate whether polar compounds generated by bacteria were skewing the
TPH-DRO analytical results, five samples were collected and analyzed for TPH-DRO with silica
gel treatment to distinguish between polar compounds and diesel-range hydrocarbons, using
Method 8015D Modified. Water samples collected from the holding tanks were analyzed using
the following methods:

DATA EVALUATION
The soil sample analytical results were compared to screening levels promulgated by the
DERR’s LUST Program. These include the conservative ISLs and the somewhat less-
conservative Tier 1 Screening Criteria that consider distance from residual contaminants to
potential receptors.
Please refer to Table 1 (Appendix B) for a summary of all soil sample results and comparative
regulatory screening levels. Exhibit 4 displays extent of excavation and soil sample locations.
Throughout the soil excavation process, several sample locations reported one or more analyte
concentrations above DERR ISLs. Upon receipt of the results, Terracon instructed DPS to
continue with the excavation process to remove impacted soils. Upon completion of the
excavation process, only two soil confirmation samples (SCEX-12SW @ 10’ and SCEX-18S @
10.5’) collected at the southwestern extents of the excavation near the property boundary
reported impacts exceeding ISLs. Further excavation in that area was not conducted due to the
property boundary and the presence of utilities.
FINDINGS AND CONCLUSIONS
The approved corrective actions effectively removed an ongoing source of petroleum
contamination to groundwater. Work performed under Work Plan IEQ-19 included removal and

proper off-site disposal of over 909 tons of petroleum-contaminated soil followed by a limited
application of chemical oxidation reagents to provide additional treatment of residual
contaminants, as well as removal, treatment, and reinjection of approximately 80,000 gallons of
free product and impacted groundwater. Based on the results of the confirmation soil samples,
a localized area of petroleum-impacted soils remains at concentrations above ISLs at the south
boundary of the extent of excavation near its southwestern corner.
With the removal of a significant portion of source material in the 2014 and 2017 excavation
areas (Exhibit 5), the residual impact to groundwater is expected to naturally attenuate.
Terracon recommends conducting quarterly groundwater monitoring to evaluate groundwater
quality conditions at the site. Once two consecutive quarters of groundwater results indicate
dissolved petroleum hydrocarbons are below ISLs, Terracon will request DERR consider“No
Further Action” status for the release.
GENERAL COMMENTS
The behavior of subsurface contaminants is a complex phenomenon involving geochemistry,
hydrogeology, and the geotechnical sciences. Terracon’s conclusions regarding the potential
for subsurface contamination are based solely upon information cited in this report. The
analyses and conclusions in this report are based upon data obtained from this assessment.
The nature and extent of variations beyond this assessment may not become evident until
further exploration. If variations then appear evident, it may be necessary to reevaluate the
conclusions of this report. The professional services provided and judgments rendered on this
project meet current professional standards and do not carry any other guarantee.

loss or damage (including attorney's fees) caused, or believed to be caused, directly or
