1.0 INTRODUCTION
1.1 Site Description
Site Name Salt Lake City Fleet Yard
Site Location/Address 800 South 325 West, Salt Lake City, Utah
General Site Description
The site was formerly used as a Salt Lake City fleet vehicle fueling and
maintenance facility. It is currently used by Salt Lake City’s Police
Department as a training facility and the Fire Department for equipment
storage.
Exhibit 1 (Appendix A) presents a topographic map of the area. Exhibit 2 is a site diagram that
indicates the approximate location of the investigation area in relation to the site.
1.2 Project Background
Exhibit 3 displays historical sample locations from previous investigations conducted from 1993
to 2013 in the vicinity of the former underground storage tanks (USTs) located on the property.
Table 1 (Appendix B) presents a summary of the results of the soil samples collected from those
sampling points, and Table 2 (Appendix B) presents a summary of the results of the groundwater
samples collected from those sampling points.
Four 10,000-gallon USTs (2 gasoline USTs and 2 diesel USTs) were removed from the site in
2011. Exhibit 4 displays the UST closure sample locations and Table 3 (Appendix B) presents
the results of the soil and groundwater samples collected during the removal event. Soil and
groundwater samples collected by IHI in 2013 (Exhibit 3; Table 1 and Table 3) identified soil and
groundwater impacts above screening levels surrounding the former tank pit after tank removal.
A corrective action plan was developed to address the petroleum-impacted soils and groundwater
remaining in the vicinity of the former UST tank pit. The selected approach included removal of
the impacted soils for off-site disposal and removal of impacted groundwater from the open
excavation, followed by a limited application of chemical oxidation reagents to treat residual
petroleum-impacted groundwater within the excavation area prior to backfilling. IHI conducted the

site work following the DERR-approved Work Plan IEQ-15. The Petroleum-Impacted Soil
Excavation Report (Terracon; November 20, 2014) documented the implementation of the Work
Plan IEQ-15 during January and February 2014. Exhibit 5 and Exhibit 6 display the soil and
groundwater sample collection locations. Table 4 presents a summary of the results of the soil
samples collected from those sampling points, and Table 5 presents a summary of the results of
the groundwater samples collected from those sampling points.
Following approved Work Plan IEQ-16 (IHI; February 26, 2015), IHI installed six new groundwater
monitoring wells (T-1 through T-6; Exhibit 7) and conducted one quarterly groundwater sampling
event (IHI; May 18, 2015) to document the concentrations of petroleum hydrocarbons remaining
in the area following the Corrective Action event. Based on the results of the first groundwater
monitoring event, concentrations of dissolved petroleum hydrocarbons were identified above
groundwater screening levels at the site, and the extent of impacts was not defined.
Work Plan IEQ-17 (IHI; May 18, 2015) was developed to further define the extent of impacts in
the vicinity of the former tank pit, as well as conducting additional post-corrective action
groundwater sampling. The Work Plan was designed to install four additional monitoring wells at
the site to further define the extent of groundwater impacts and collect soil samples during the
installation of the wells to further define the extent of soil impacts. This report documents the
results of the installation of the four additional monitoring wells, the analytical results from the soil
samples collected during monitoring well installation, as well as the analytical results from two
rounds of groundwater sampling conducted at the site.
1.3 Scope of Work
The objective of the Work Plan was to further delineate soil and groundwater impacts in the vicinity
of the former tank pit, as well as to conduct additional groundwater sampling events at the site.
The project goal is to meet regulatory limits, less than Initial Screening Levels (ISLs) for
unrestricted land use, and receive a “No Further Action” status for Release ID IEQ.
1.4 Standard of Care
Terracon’s services were performed in a manner consistent with generally accepted practices of
the profession undertaken in similar studies in the same geographical area during the same time
period. Please note that Terracon does not warrant the work of laboratories, regulatory agencies
or other third parties supplying information used in the preparation of the report. These services
were performed in accordance with the scope of work agreed with you, our client, as reflected in
our proposal, and were not restricted by ASTM E1903-13.
1.5 Additional Scope Limitations
Findings, conclusions and recommendations resulting from these services are based upon
information derived from the on-site activities and other services performed under this scope of

work. Such information is subject to change over time. Certain indicators of the presence of
hazardous substances, petroleum products, or other constituents may have been latent,
inaccessible, unobservable, non-detectable or not present during these services, and we cannot
represent that the site contains no hazardous substances, toxic materials, petroleum products, or
other latent conditions beyond those identified during this investigation. Subsurface conditions
may vary from those encountered at specific borings or wells or during other surveys, tests,
assessments, investigations or exploratory services; the data, interpretations, findings, and our
recommendations are based solely upon data obtained at the time and within the scope of these
services.
1.6 Reliance
This report has been prepared for the exclusive use and reliance of the Salt Lake City Corporation
and government entities having jurisdiction over the site. Use or reliance by any other party is
prohibited without the written authorization of the Salt Lake City Corporation and Terracon.
Reliance on the report by the client and all authorized parties will be subject to the terms, conditions
and limitations stated in the proposal, report, and Terracon’s Terms and Conditions. The limitation
of liability defined in the Terms and Conditions is the aggregate limit of Terracon’s liability to the client
and all relying parties.
2.0 LIMITED SITE INVESTIGATION
2.1 Environmental Soil Borings
Prior to drilling, the public utility location service (Blue Stakes) was notified at least 48 hours prior
to drilling as required by law. In addition, a subcontracted private utility location service was used
to identify and mark subsurface utilities in the area of each boring location.
Terracon’s soil boring and monitoring well installation activities were conducted on June 10, 2015,
by Amy Findley, Utah UST Groundwater and Soil Sampler (#GS-1465), and David Jamison, Utah
UST Groundwater and Soil Sampler (#GS-1710). The approximate boring locations, T-7 through
T-10, are shown on Exhibit 7 (Appendix A).
The borings were advanced using direct push drilling equipment. Mechanized drilling services
were performed by a State of Utah-licensed Monitoring Well Driller, using a direct push drilling rig
under the supervision of a Terracon Environmental Project Manager. Mechanized drilling
equipment was cleaned using a high-pressure washer prior to beginning the project and before
beginning the soil boring. Soil samples were collected continuously from the boring using a five-
foot macro-core sampler.
Soil cores were observed to document soil types, color, moisture content, and sensory evidence
of environmental impacts. The soil samples were also field-screened using a portable

photoionization detector (PID) – Mini Rae 2000 PID to determine the potential presence of total
volatile organic compounds (TVOCs). The subsurface soil types encountered during sample
collection varied between borings but generally consisted of sands, clays, and gravels. PID
readings ranging from non-detect to 6.0 parts per million (ppm). Groundwater was typically
encountered at depths of approximately 9 to 10 feet below ground surface (bgs). Detailed
lithological descriptions and PID readings are presented on the soil boring logs included in
Appendix C.
2.2 Subsurface Soil Sampling
One soil sample was collected from each boring from the interval displaying the highest impacts,
or, if impacts were not observed, from the soil-groundwater interface.
2.3 Monitoring Well Installation
The four borings were completed as permanent groundwater monitoring wells (T-7 through T-10),
using the following procedures:
n Installation of 10 feet of 2-inch diameter, 0.010-inch machine slotted PVC well screen with
a threaded bottom cap;
n Installation of 2-inch diameter, threaded, flush-joint PVC riser pipe to the surface;
n Addition of pre-sieved 20/40 grade silica sand for annular sand pack around the well
screen from the bottom of the boring to approximately 2 feet above the top of the well
screen;
n Placement of 2 feet of hydrated bentonite pellets above the sand pack;
n Addition of cement/bentonite slurry to the surface; and,
n Installation of a 7-inch diameter, circular, bolt-down, steel, monitoring well cover with a
locking well cap inset in a flush-mount concrete well pad.
2.4 Groundwater Sampling
On June 12, 2015, Mark Lilly (Utah UST Groundwater and Soil Sampler #GS-449) measured
groundwater levels in the four newly installed monitoring wells T-7 through T-10. The wells were
then developed by surging and removing groundwater until the well discharge was relatively free
of fine-grained sediment. Prior to sample collection, each well was purged of a minimum of three
casing volumes (or until the well ran dry and was allowed to recharge), using a new disposable
bailer for each well. Groundwater samples were then collected directly into the appropriate
laboratory-provided containers.
On September 10, 2015, Jeff Kolmel (Utah UST Groundwater and Soil Sampler #GS-1592)
returned to the site to collect additional groundwater samples from monitoring wells T-1 through
T-10, as well as MW-4 and MW-5. Prior to sample collection, each well was purged of a minimum

of three casing volumes (or until the well ran dry and was allowed to recharge), using a new

appropriate laboratory-provided containers.
2.5 Gradient Measurement
Based on the water level measurements and top-of-casing elevation data collected from
monitoring wells T-1 through T-10, and MW-4 and MW-5 on September 10, 2015, the flow
direction within the former UST basin was to the south- southeast (Appendix A; Exhibit 7).
3.0 LABORATORY ANALYTICAL METHODS
The soil and groundwater samples were collected and placed in laboratory-prepared containers,
labeled, and placed on ice in a cooler. The samples and completed chain-of-custody forms were
shipped via overnight courier to Environmental Science Corporation analytical laboratory in Mt.
Juliet, Tennessee (a Utah Certified Laboratory).
All samples were analyzed by laboratory test methods as appropriate for the type(s) of
contaminants most likely to be associated with the former UST systems: methyl tert-butyl ether
(MTBE), benzene, toluene, ethylbenzene, xylenes, and naphthalene (BTEXN), total petroleum
hydrocarbons in the gasoline range (TPH-GRO), and total petroleum hydrocarbons in the diesel
range (TPH-DRO). Samples were analyzed using the following methods:

4.0 DATA EVALUATION
Please refer to Table 4 (Appendix B) for a summary of the laboratory analytical results for
historical excavation confirmation soil samples and soil samples collected during this
investigation. Please refer to Table 5 (Appendix B) for a summary of the laboratory analytical
results for groundwater samples. Tables 4 and 5 include regulatory screening levels for
comparative purposes.

4.1 Soil Sample Results
Soil samples collected during the installation of monitoring wells T-6 through T-10 did not report
petroleum compounds above ISLs.
4.2 Groundwater Sample Results
Groundwater samples were collected from T-7 through T-10 in June. Analytical results reported
dissolved petroleum hydrocarbons above the laboratory reporting limit in wells T-8 (TPH-DRO,
MTBE), T-9 (TPH-DRO, TPH-GRO, MTBE), and T-10 (TPH-DRO), but below their respective
ISLs. The concentration of benzene reported in T-8 was the only constituent to exceed an ISL.
None of the groundwater concentrations reported exceeded Tier 1 screening levels.
Groundwater samples were collected from T-1 through T-10, and MW-4 and MW-5 in September.
Analytical results reported low concentrations of dissolved petroleum hydrocarbons present in all
12 wells sampled.
Only wells T-3, T-4, T-6, T-8 and T-9, located in the immediate down-gradient direction of the
excavation area, reported constituents above an ISL. Only concentrations of TPH-DRO and
benzene exceeded the ISL, and no concentrations reported exceeded a Tier 1 screening level.
The highest concentration of TPH-DRO was reported at 1.87 mg/L in T-6, and the highest
concentration of benzene was 0.104 mg/L in T-3.
5.0 FINDINGS AND CONCLUSIONS
Based on the results of this limited subsurface investigation, petroleum impacts to soil originating
from the former tank pit south of the former tank basin were not reported above ISLs, and it
appears that the extent of the remaining impacted soil exceeding ISLs have been removed or
remediated.
Based on the results of the two quarterly groundwater sampling events, it appears that dissolved
petroleum hydrocarbons, specifically TPH-DRO and benzene, remain above ISLs south of the
former tank pit.

All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times. All MDL (LOD) and RDL (LOQ)
values reported for environmental samples have been corrected for the dilution factor used in the
analysis. All Method and Batch Quality Control are within established criteria except where addressed
in this case narrative, a non-conformance form or properly qualified within the sample results. By my

Abbreviations and Definitions
SDG Sample Delivery Group.
MDL Method Detection Limit.
RDL Reported Detection Limit.
ND,U Not detected at the Reporting Limit (or MDL where applicable).
RPD Relative Percent Difference.
(dry)Results are reported based on the dry weight of the sample. [this will only be present on a dry
report basis for soils].
Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD)
from a quality control sample. The Original Sample may not be included within the reported SDG.
(S)Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control
Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring
recovery. Surrogates are not expected to be detected in all environmental media.
