1 Purpose
1.1 This work instruction is to document the site process for reviewing, auditing, and
maintaining canceled confined space permits. The purpose of this process is to
ensure proper protocols and procedures are being followed for confined space
entries.
2 Process for reviewing, auditing, and maintaining canceled confined space permits
2.1 After a confined space entry has occurred, the completed permit is forwarded to
the safety and health department for review.
2.2 All permits will be maintained for the current year with the Safety and Health
Department. The permits will be maintained as employee exposure records.
2.3 Any deficiencies identified during this initial review may require training with the
entry supervisor, and employees who affected the confined space entry and/or
procedure/work instruction modification.
2.4 Annually the Safety and Health Department or designee will spot check the
completed permits as part of an evaluation of the overall confined space program.
This process will be documented in the Clive surveillance program.
