Environmental Engineer III/Review Engineer Utah Division of Drinking Water Subject: Operating Permit Request for Old Highway 40 Pump Station and Water Treatment Plant (TP009), System #26086, DDW File #12044 Dear Julie: We would like to request approval for an operating permit for the subject project. The contractor, Ralph L. Wadsworth Construction, has completed all of the construction activities for the project. The bacteria tests have been completed. The operating permit check list is addressed below. The following seven items address the conditions that must be met for compliance with Utah Administrative Code R309-500-9.3 to receive an operating permit. a. Certification of Rule Conformance by a professional engineer that all conditions of Plan Approval were accomplished and if applicable, changes made during construction were in conformance with rules R309-500 through 550. Bowen Collins & Associates performed construction observation throughout the construction period. Bowen, Collins & Associates certifies that, insofar as can be observed, the construction of the work conformed to the approved plans and the drinking water rules. There were minor changes made during construction and all changes were installed in conformance with the UAC rules. b. As-built or record drawings incorporating all changes to approved plans and specifications, unless no changes are made from previously submitted and approved plans during construction. A link to access the final record drawings for the project can be supplied upon request. This letter shall serve to provide documentation that, for the Old Highway 40 Pump Station and Water Treatment Plant, the approved design drawings were modified by Bowen, Collins & Associates per the inspector’s and the construction contractor’s redlined notes for the water improvements installation. The modified plans will serve as the record drawings for the project. c. Confirmation that a copy of as-built or record drawings has been received by the water system owner. Evidence of proper flushing and disinfection in accordance with the appropriate ANSI/AWWA Standard. The bacteria test results have been supplied to the Division by Jordanelle Special Service District. Jordanelle Special Service District sampled the water and were then analyzed by Chem-Tech Ford Laboratory. All flushing and disinfection was accomplished in accordance with the latest applicable standards. e. Where appropriate, water quality data. All available water quality data has been provided to the Division of Drinking Water by Jordanelle Special Service District. The sample results from the radio nucleotide testing have not been received yet, but will be provided to the Division once they are available. f. All other documentation which may have been required during the plan review process. N/A g. Confirmation that the water system owner has been provided with an Operation and Maintenance manual for the new facility. An O&M manual for this facility has been compiled by the Contractor and Engineer and has been supplied to Jordanelle Special Service District. This should address all requirements of the plan approval for the project and for the operating permit.
