Dear Darren Hess:The Division of Drinking Water (the Division) received a request for an Operating Permit for Segment 4, 12th Street Water Line Replacement from your consultant, Joshua
Prettyman, PE with CRS Engineers on July 22, 2025. The Division issued Plan Approval for this project on August 29, 2023.
Our understanding of the project was the replacement of the 12th Street pipeline along 900 South
between approximately 7300 West and 10000 West with 30-inch diameter PVC C900 DR-18 water line.
We have received the following information for Segment 4, 12th Street Water Line Replacement (DS001):
Certification of Rule Conformance with plan approval conditions by a professional engineer.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Segment 4, 12th Street Water Line Replacement is hereby issued
as constituted by this letter. You may now place Segment 4, 12th Street Water Line Replacement in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
